0127298831 2017-09-14 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to five 400 barrel production fluid holding tanks. Production records in RBDMS are current to 2017.
0240806215 2018-04-25 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains five 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 1246 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 450 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.
0303200957 2016-02-15 Completion Testing No Violations I received a call from the Emergency Response section of the division. E.P.A had been notified of a spill that occurred on this location. Upon receiving the call I contacted the operator. The spill consisted of 300 barrels of water on containment and 50 barrels off containment. The remediation was already underway when I received the call.

0512545676 2018-08-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 4-string casing design and was in normal production. Annular pressure monitoring was in place on both the 9.625-inch diameter casing gauge read 1415 pounds per square inch, the 13.375-inch diameter casing gauge read 300 pounds per square inch. Production equipment included an individual Gas Processing unit, and 5- 400-barrel production fluid tanks shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current.
0515642081 2018-12-05 Production Wells No Violations Status check inspection found well pad surface and lease road of stone construction and in good repair. The well pad site does have an earthen berm that appears to be sufficient and preforming with vegetation established. No movement of the outward slopes was observed. There lease road is equipped with a gale and signage is present at the entry point of the lease road along with being posted on each well head. There is a 3rd party midstream point on the left rear corner of the well pad that consists of a contact tower and glycol heater. There are 5 wells on the pad with 5 processing units. All 5 wells were equipped with riser pipes and transducers on the A&B sections for annular pressure monitoring. The production tree valves are open. The production fluid facility consists of 5 steel construction 400 barrel fluid capacity tanks situated inside a polymer lined steel containment dike area. Vegetation and erosion was observed on all outward facing slopes and all earthen stock pile areas.
