1007824152 2018-07-18 Production Wells No Violations Status check inspection accompanied by ODNR Inspector Ryan Armstrong found the well head isolated while stimulation operations are taking place on the # 7 and # 5 wells on the well pad. The well head has had VR isolation plugs set in being the production valves and a back pressure valve in the tubing hanger. A cage is installed over the well head. A production facility is present on the rear left corner of the well pad.

1480968331 2015-01-13 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the conductor pipe. I was informed by XTO consultant, Paul Hahn, that Express Energy Services drilled a 36 inch diameter hole to a depth of 100 feet. Express set 100 feet of 106lbs/ft, 29.25 ID casing. Express pumped 275sk of Class A cement, 15.6ppg, 1.18 yield, 2% CaCl2, 0.25lbs/sk of flake. Express displaced with 64.8bbl of fresh water. 25bbl of cement returned to surface and stayed at surface while I was on site.
1483192222 2016-12-31 Production Wells No Violations On site inspection as a follow up to a call received by the Divisions Emergency Response Supervisor Trish Cartmell at 5:20 am on this day in regards to a Glycol Heater fire on the well pad. Upon my arrival the fire had been extinguished and all active wells on the pad shut in and isolated. XTO Company Man Tracy VanScoy was on site and reported the well drilling and completion process has be completed and the well is in its initial flow back production stages. Mr. VanScoy pointed out the dump valve on the line heater had washed out and allowed gas to escape. The escaping gas was then ignited by the internal flame on the line heater. The rear area of the line heater was damaged by the fire along with a 2nd line heater next to it servicing the other operating well on the pad. The fire appeared to be contained only to the area of the heaters with only the line heaters and polymer containment damaged by the fire. The line heater for the 2-H well was leaking glycol. The glycol was being recovered by placing a bucket under the damaged area that was leaking. The company has vacuum trucks enroot to recover the remainder of the glycol in the heater. There is also (3) 400 barrel production tanks inside the containment with the glycol heaters that were not affected by the event. A preliminary boundary inspection was conducted and no product was observed outside the production containment area.
1689609185 2018-10-16 Production Wells No Violations On the day of my inspection I witnessed that the well was currently shut in and annular pressure monitoring was in place, at the time of my inspection Key Energy Services had pressure pumping equipment hooked up to the well head. Annular pressures were monitored with transducers without a digital readout so could not be verified. Both the 9.625-inch diameter casing and 13.375-inch diameter casing were tied to a pressure bleed down line and the 13.375-inch diameter casing was open at the time of my inspection. Production Equipment included a gas processing unit and 6- 400-barrel steel production fluid tanks. There was a Key Energy Services coil tubing rig and pressure pumping truck on location at the time of my inspection. After speaking with XTO Production superintendent Tracy Vanscoy he informed me that Key Coil Tubing Rigs blow out preventer malfunctioned, and they were currently waiting on a new Blow Out Preventer which is scheduled to arrive on Friday 10/19/2018.
