0326742692 | 2017-10-26 | Production Wells | No Violations | This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains five 400-barrel production fluid holding tanks. Production records for this well are current to 2017 within RBDMS. |

1079440861 | 2018-09-25 | Production Wells | No Violations | On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing with a plunger lift system. Annular pressure monitoring was in place the 9.625-inch diameter intermediate casing gauge read 0 pounds per square inch, the 13.375-inch diameter surface casing gauge read 105 pounds per square inch. Production equipment included a sand trap, Line Heater, Gas Processing Unit. There were also 5- 500-barrel steel production fluid tanks inside secondary containment, that were shared with the rest of the producing wells on the pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was current in RBDMS. |
1102181716 | 2019-02-07 | Preliminary Restoration | No Violations | Preliminary Restoration found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad surface and lease road is stone construction and in good repair. The lease road is gated and the well pad has a perimeter earthen berm with a 8 chain link fence on the berm. Signage is posted at each of the 8 wells on the pad. All 8 wells have artificial lift production equipment installed on the production tree. Annular pressure monitoring is in place on the well head A&B sections. The production facility consists of 5 steel tanks capable of holding 500 barrels fluid volume each, the tanks are situated inside a polymer lined steel containment dike area. 8 gas processing units are present on the well pad in 2 different locations. |
1295152306 | 2014-10-06 | Casing String Witnessed | No Violations | Onsite with Inspector Andrew Corder to witness the cementing of the mine string. Alpha Hunter rig drilled a 28 hole to 210. Mine void encountered at 163-168. 24 casing was set to 203. Cement baskets set at 125 and 145 per company man. Allied cementing was onsite with 195 sks for the bottom balance job. Rig had difficulty running casing and delayed job. Andrew stayed to view top out job. |
