0136376338 2018-05-02 Casing String Witnessed No Violations On the day of my inspection I witnessed that Appalachian Drilling had ran 1 inch diameter tubing to a depth of 430 feet and tagged cement. C&J Energy Services rigged up established circulation and pumped 68 barrels of cement with good cement returns to surface at 62 barrels, with 6 barrels of excess at surface.
0207008298 2018-10-22 Completion Testing No Violations Status check inspection found there are 7 wells on the pad 5 which were previously completed and 2 that were recently drilled and stimulated. These 2 wells are awaiting the completion drill out process. The 2 wells recently completed were the B-8-H and B-6-H. These wells are equipped with a 7.0625 diameter manually operated atop the B section and flow cross. Atop the 1st master valve is a flow cross followed by a second master then the pressure pumping attachment point (goat head). Annular pressure monitoring is in place on the A & B section. Crews are on site mopping up from the stimulation. The production tank farm is on the rear left hand corner of the pad and consists of a total of 5 steel tanks capable of holding 400 barrels capacity each. The pad is of stone construction and in good repair.
0299536372 2018-07-12 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Patterson UTI 579 was currently tripping in the hole on a fishing retrieval run. After speaking with company man Gary Moore he informed me that they had drilled to 7,857 feet and in the process of making a connection the drill string separated at the Gap Sub at approximately 7,758 feet. Patterson UTI was currently at 5,800 into the well bore when I arrived on location. XTO Company Man Gary Moore informed me that if they were unsuccessful in their retrieval that they plan to plug back and deviate around the bottom hole assembly. While on location I confirmed that the producing wells on the well pad were shut in and annular pressure monitoring was in place with valves in the open position. I told Gary to keep me informed with any progress made on the well to send me all plans for plugging back. There were no violations at the time of my inspection.
0441272163 2018-07-18 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Patterson UTI 579 had drilled the 12.25-inch diameter wellbore to 9005 feet. When running the 9.625-inch diameter casing the casing got hung up at 8663 feet. Patterson 579 was unable to break circulation at this depth and KLX Energy Services was called to location to perforate the 9.625-inch diameter casing at 8605 feet. KLX Energy Services performed an 8 shot perforation using .5-inch diameter shots at 4 shots per foot. After casing was perforated Patterson 579 was able to establish circulation at 4.5 barrels per minute, with pressures at approximately 280 pounds per square inch. After speaking with XTO Company man Jr. Hill he informed me that they would continue to circulate on the well bore and attempt to free casing. I informed Jr. Hill that they needed to submit a plan for future operations to the me.
1146361755 2018-04-23 Casing String Witnessed No Violations On site to witness the casing integrity test. The company loaded the hole with water and pressure tested the casing with 300 pounds per square inch of pressure and held for 30 minutes. The casing integrity test was successful.
1400865248 2018-05-09 Casing String Witnessed No Violations On the day of my inspection I witnessed that Appalachian Drilling had drilled the 17.5 inch diameter hole to a depth of 2460 feet. Appalachian Drilling ran 2428 feet of 13.375 inch diameter casing with 533 feet of 61 pound per foot, and 1895 feet of 54.5 pound per foot J-55 Casing. Centralizers where ran on every joint of casing. With 2 feet above KB, KB height is 13 feet above ground level. C&J Energy Services was on site and rigged up their cement head, loaded lines with 5 barrels of fresh water and pressure tested to 3000 pounds per square inch. C&J Energy Services broke circulation with 385 barrels of fresh water, pumped 50 barrels of gel, 5 barrels of water, and 553 barrels of Class A cement at 15.6 pound per gallon and 1.22 cubic feet per sack with 5% chlorides, .25 pounds per sack flake, .5% accelerator, 1% CJ 101 gypsum, .2% defoamer, and .15% cement retarder. Cement was displaced with 367 barrels of fresh water. Plug was bumped at 1025 pounds per square inch and pressured up to 1525 pounds per square inch and held for 5 minutes, floats held with 2 barrels of flow back. Cement returned to surface at 215 barrels into displacement with 150 barrels of excess cement to surface. Informed the company man that casing must remain static for 8 hours or 500 pounds per square inch compressive strength before further actions could be taken.
1523886264 2018-04-15 Casing String Witnessed No Violations Lightning Energy was on site and had reached the total depth of the 36-inch diameter open hole section of the well bore at 100 feet. The 30-inch diameter casing was ran to total depth. C & J Cementing Services was on site to cement the conductor casing in place with 66 barrels of Class A cement and 74 barrels of displacement was pumped. Cementing operations produced cement returns to surface.
1525983425 2018-05-01 Casing String Witnessed No Violations On the day of my inspection I witnessed that Appalachian Drilling had drilled the 17.5 inch diameter hole to a depth of 2510 feet. Appalacian ran 2397 feet of 13.375 inch diameter casing with 506 feet of 61 pound per foot, and 1891 feet of 54.5 pound per foot J-55 Casing. Centralizers where ran on every joint of casing. With 3.6 feet above KB, KB height is 13 feet above ground level. C&J Energy Services was on site and rigged up their cement head, loaded lines with 5 barrels of fresh water and pressure tested to 2500 pounds per square inch. C&J Energy Services broke circulation, pumped 50 barrels of gel, 5 barrels of water, and 390 barrels of Class A cement at 15.6 pound per gallon and 1.22 cubic feet per sack with 5% chlorides, .25 pounds per sack flake, .5% accelerator, 1% CJ 101 gypsum, .2% defoamer, and .15% cement retarder. Cement was displaced with 368 barrels of fresh water. Plug was bumped at 750 pounds per square inch and pressured up to 1250 pounds per square inch and held for 5 minutes, floats held with 1.5 barrel of flow back. There were no visible cement returns to surface. After the 8 hour set time Company man plans to remove slips and run 1 inch diameter tubing and try and tag top of cement. I explained if could not get past centralizers then Bond Log will have to be run to confirm cement top. C&J Energy Services believed that they had most of there 50 barrel gel back to surface and cement would be just below surface.
