0068938096 2019-02-25 Production Wells No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and no erosion control issues noted. The well pad and lease road surface are stone construction and in good repair. The well pad is not equipped with a perimeter berm. (2) wells were recently drilled the Pavich #3 and # 5 and completed on the pad with (2) additional pre existing wells present. The (2) newer wells identified as the # 3&#5 are producing off the 5.5 diameter production casing through a primary master valve and production tree and were recently turned over from flow back to production. Annular pressure monitoring is in place on the well head A&B sections. There is (2) tank battery production facilitys present with the well pad with the facility associated with the # 3&#5 wells equipped with (4) steel 400 barrel tanks that are situated inside a polymer lined steel containment dike area.
0951531748 2018-07-12 Casing String Witnessed No Violations On the day of my inspection I witnessed that Patterson Rig 579 had drilled the 24-inch diameter well bore to a depth of 725 feet. Wyoming Casing crew ran 701 feet of 18.625-inch diameter J 55 buttress thread casing at 87.5 pound per foot, with 2.4 feet of casing above Kelly Bushing, which is 26 feet above ground level. Two cement baskets were set at 569 feet. C&J Energy Services was on site and rigged up their cement head and loaded lines with 5 barrels of fresh water and pressure tested to 1500 pounds per square inch. C&J Energy Services pumped a 100-barrel gel spacer with poly flake, washed up and then pumped 311 barrels 1456 sacks of 15.6 pounds per gallon Class A cement with a yield of 1.2 cubic feet per sack mixed with 5.25 gallons of water per sack. Cement was displaced with 201 barrels of fresh water, with 105 barrels of good cement to surface. Plug was bumped at 340 pounds per square inch and pressured up to 840 pounds per square inch then held for 5 minutes with .5 barrels of flowback. I informed XTO company man JR Hill that the casing must remain static until cement reaches a compressive strength of 500 pounds per square inch.

1407521654 2018-08-01 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that the Cellars and conductors had been set and was currently under a steel plate. I spoke with XTO representative Paul Hahn and he informed me that the cement jobs went according to plan, and cement did no drop after 1 day of setting, this could not be confirmed due to the cellars being covered. Well was spudded and cemented on 7/27/2018.
