0188688537 2018-08-16 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design, 5.5-inch diameter production casing that appeared to be open flowing. Annular pressure monitoring was in place on both the 13.325-inch diameter surface casing and the 9.625-inch diameter deep intermediate casing, pressures were monitored with transducers with no ability to read pressures. Both casings had bleed down lines ran to the production fluid tanks, at the time of my inspection the 13.375-inch diameter casing was open flowing into tanks. Production equipment included a vertical separator and tank vault de-foaming unit. There were also 2- 400-barrel production fluid tanks inside secondary containment. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
1239899025 2015-01-22 Preliminary Restoration No Violations The preliminary restoration for this well is completed. The pad is in satisfactory condition with proper erosion and sedimentation controls in place. Company shall continue to monitor the location to ensure the pad remains in satisfactory condition. Preliminary restoration passed.
1399628931 2014-05-09 Casing String Witnessed No Violations On the day of my inspection Sidewinder 08 had drilled the surface hole. The 24 surface casing was ran and cemented. See cement ticket for details. No violations noted.
1399635178 2014-05-09 Drill / Deepen / Reopen No Violations On the day of my inspection I inspected the next casing string to be ran. It was new 18.625 casing that appeared to be of satisfactory condition. No violations noted.
1513040525 2017-08-23 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to a production area and to sales. The production area contains two 400-barrel production fluid holding tanks and a vertical mechanical separator. Production records in RBDMS are current to 2017.
1543337175 2018-11-27 UrbanProduction Wells No Violations Status check inspection found one well present on the pad that is currently in production. The well is equipped with an lower A and upper B section. Attached to the lower A and upper B section well head outlet ports are 2 diameter riser pipes, the pipes are equipped with valves and gauges to monitor annular pressure. The tanks are tied into a 2 sales line that runs back to the production tanks. There is a production tree present on the B section with 3 manual valves and one emergency shut down hydraulic valve present. All pressure monitoring is done through electronic transducers. The production facility consists of (2) streel construction 400 barrel fluid volume tanks situated along with a mechanical separator and gas metering equipment inside a polymer lined steel containment dike area. The production facility is on a separate pad adjacent to the well pad that is also at the entry point of the well pad. There are emergency shut down devices situated at the well pad entry point and tank battery only. Also on the right side of the lease road at the well pad entry point is the 3rd party midstream operator. Also noted is that the well pad and production pad do not have perimeter earthen berms present.
