0351653041 2018-04-06 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a plunger lift system with valves in the open position. Gauges were present for monitoring annular pressure 13.375 inch diameter casing digital readout was not working at time of my inspection, 9.625 inch diameter casing read 1982 pounds per square inch, there was a blow down line installed on the 9.625 inch diameter casing, valve was currently shut. The production equipment included a Gas Processing unit, a line heater and a sand trap. Also present 5- 400 barrel purge tanks shared with 3401320746, 3401321072, 3401321073, 3401321074, 3401321075, 3401321076. Identification tags were present and production was up to date.

1534265265 2018-08-13 Production Wells No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that the well was equipped with a four-string casing design. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 1400 pounds per square inch, the 13.375-inch diameter surface casing gauge read 426 pounds per square inch. The 5.5-inch diameter production casing was also at 1400 pounds per square inch and appeared to be communicating with the 9.625-inch diameter deep intermediate casing. The 2.875-inch diameter tubing and flow line pressures were at 1475 pounds per square inch. Production equipment included an individual Gas Processing unit. Shared production equipment included 5- 400-barrel production fluid tanks shared with 34013210760000, 34013210750000, 34013210740000, 34013210730000, 34013210720000, 34013207460000. The well pad and all equipment were contained in an earthen dike with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.
1535044828 2018-08-23 Production Wells No Violations Follow up inspection to violation # 1543862543 issued on 08/13/2018. Upon inspection the well was observed to be equipped with a lower A and upper B section with 2 diameter riser pipes attached to the A&B section outlet ports, the riser pipes are equipped with valves and gauges. The A section gauge which monitors the pressure between the 13.375 and 9.625 diameter casing string reads 478 pound pressure. The B section gauge which monitors the pressure between the 5.5 and 9.625 diameter strings reads 750 pounds pressure. The 5.5 diameter production casing reads 1,500 pounds pressure and the production tubing head reads 1,500 pounds pressure. 7 wells total are producing on this pad. The production fluid storage area consists of 5 steel design 400 barrel fluid volume tanks that are situated inside a polymer lined steel containment dike area.
1536773010 2018-09-12 Production Wells No Violations Follow up inspection to violation # 1543862543 issued on 08/13/2018 found a service rig snubbing unit owned by Consolidated Well Services on site and rigging up preparing to pull the production tubing out of the well bore. A 2 way check back pressure valve has been installed in the tubing hanger and the production tree removed. A 7.0625 diameter manual master valve has been installed on the well head B section with a work over blow out preventer being installed above the master valve. The company man on site indicated this rig is to only pull the production tubing at this time with this rig.
1537976240 2018-09-26 Production Wells No Violations Compliance notice follow up inspection to violation # 1543862543 found the company has pulled the production tubing out of the well. No further repairs or evaluation was reported as completed past that point. The production tubing head has been removed from the well and a 7.0625 master valve installed. The valve is in the closed position with no flow line attached. There is 2 diameter riser pipes with valves and gauges present attached to the well head A&B sections. The A section which monitors the 13.375 casing pressure shows 34.22 pounds pressure. The B section which monitors the 9.625 casing pressure shows zero pressure. The 5.5 casing shows 1,600 pounds pressure. There is a flow line attached to the B well head section. Polymer containment is present on the well pad and this is a 7 well pad. On this day I called John Ellinder with EQT and he indicated the tubing was pulled and a bond log ran. Mr. .Ellinder also indicated a pressure test was conducted on the casing and the production tree was found to be leaking. I requested a copy of the bond log.
1542809798 2018-11-21 Production Wells No Violations Compliance notice follow up inspection to violation # 1543862543 found the company has made corrections to the well to remedy the annular pressure. The company has submitted email documentation referencing information pertaining to the annular pressure issue being caused by pressure leaking through the well head 5.5 diameter casing seal allowing annular pressure to be present between the exterior of the 5.5 diameter production casing and the internal area of the 9.625 diameter casing string. I observed a new well head B section tubing head adaptor plate has been installed and (2) manual gate valves and (2) 1/4 turn ball valves are present on the production tree. There is also a motor valve present on the production flow line, the motor valve control line that controls the motor valve is tied back into the 5.5 casing well head section outlet. Annular pressure monitoring is in place and upon inspection 18 pounds pressure was found on the 9.625 section and zero pressure was found on the 13.375 well head section. The company is submitting the well pressures to the Division. The previous violation is considered resolved.
1543862543 2018-08-13 Production Wells Violations Noted Status check inspection accompanied by ODNR Inspector Ryan Armstrong. The purpose of inspection was to check the well head pressures. The well is equipped with a lower A and upper B section with 2 diameter riser pipes installed on the A&B sections to monitor annular pressure. The riser pipes on both the A&B sections are equipped with valves and gauges. The A section which monitors the pressure between the 13.375 and 9.625 diameter casing strings shows 426 pounds per square inch (psi) on the manual gauge. The B section which monitors the pressure between the 9.625 and 5.5 diameter casing strings shows 1,400 psi on the gauge. The 5.5 diameter casing internal pressure shows 1,400 psi on the gauge. The production tubing head shows 1,475 psi going into the flow line. On the East side of the well pad is a production facility consisting of 5 steel construction 400 barrel capacity production tanks situated inside a polymer lined steel containment dike area. The well pad is protected by an earthen berm however no sump drains with gate valves are present. Emergency shut down devices are placed at all critical points on the well pad with fire extinguisher stations present, however all fire extinguishers are missing. Also noted was during my last inspection on 07/10/2018 the 5.5 production casing internal working pressure and annular pressure was equalized at 2,214 pounds per square inch pressure. Historically addressing the well history. On June 18, 2018 the Division received email notification from John Helmick with EQT indicating that sometime on or after 3/19/2015 the well was put into production and higher than expected annular pressures were observed. Also noted was the company indicated that a hole has developed in the 5.5 diameter production casing. Corrective action required: Under OAC 1509:12 Defective casing or Tubing and OAC 1501:9-1-08 Well construction rules section (N) Annular Pressure. The well owner/operator shall contact inspector Bill Stolarik within 7 days
1672412371 2018-04-06 Production Wells No Violations On the day of my inspection I witnessed that the well was open flowing with no artificial lift system in place, valves were in the open position. Gauges were present for monitoring annular pressure 13.375 inch diameter casing digital readout was not working at time of my inspection, 9.625 inch diameter casing read 1982 pounds per square inch, there was a blow down line installed on the 9.625 inch diameter casing, valve was currently shut. 1693031631 2018-07-10 Production Wells No Violations On site inspection accompanied by ODNR Inspector Ryan Armstrong. The well site entrance point was marked with signage and the lease road was of gravel construction. There is 2 well pads sitting next to one another back the same lease road with the Gold Digger being the rear pad and the Sonuvadigger being the pad closest to State Route 9. There was recently 5 wells newly drilled on the pad and 2 prior producing wells. The said well was equipped with electronic annular pressure monitoring on both the 13.375 and 9.625 well head sections. The 9.625 well head gauge showed 2,214 psi and the 13.375 showed 389 psi on the manual gauge also the 5.5 production casing had 2,214 psi. The well was selling off the production tubing tree with 2 manually controlled valves in place on the well head followed by an emergency shut down valve with multiple shut down point on the well pad. Also present on location is one gas processing unit present for each well supported by a production fluid facility consisting of 5 steel tanks of 400 barrel construction design situated inside a polymer lined steel containment dike area.
