0412804968 2014-02-13 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing being run and cement. See ticket for details. No violations noted.
0419801269 2018-02-13 UrbanProduction Wells No Violations On the day of my inspection the well was equipped with 5.5-inch diameter production casing. The well appeared to be free flowing with no artificial lift technology present. The well was piped to a gas processing unit then to a production facility off to the side of the pad. The production facility was fenced in and contained two 400-barrel production fluid holding tanks. The location that contained the well was separately fenced in as well. Production records within the RBDMS are current to 2017.
0560428029 2016-10-06 Production Wells No Violations This well is currently equipped properly to be considered capable of production. The piping from the well leads to a gas processing unit and then to the tank battery. The tank battery consists of two 400 barrel production fluid tanks. The site appears to be in satisfactory condition.
0612019660 2017-08-21 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well site area is fenced in. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to a production area and to sales. The production area contains the gas processing units and two 400-barrel production fluid holding tanks a contact tower and another gas processing unit that is with a fenced in area. Production records in RBDMS are current to 2017.
0742611466 2018-08-23 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 4-string casing design with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place on the 13.375-inch diameter surface casing gauge read 500 pounds per square inch, the 9.625-inch diameter casing gauge read 0 pounds. Production equipment included a Sand Trap, a line heater, and vertical fluid separator. There are two 400-barrel production fluid tanks, a second line heater, fuel gas scrubber, charcoal filters, vertical 3 phase separator, coalescent filter, and horizontal separator shared with the other well on the pad. There were multiple Emergency Shut Down devices on location, Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
1721376276 2016-10-03 Production Wells No Violations This well is currently equipped properly to be considered capable of production. The piping from the well leads to a gas processing unit and then to the tank battery. The site appeared to be in satifactory condition.
1769902190 2014-05-05 Drill / Deepen / Reopen Violations Noted On the day of my inspection there was brine contaminated water escaping the drilling pad. Samples and photos were taken. Hess Ohio Resources shall remediate all necessary areas to ensure that all contamination has been removed and proper vegetation established to prevent erosion. Company has completed the remedial work for this location and is now in compliance. Compliance Action RESOLVED.
