0222008602 2018-04-09 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 25 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 210 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.

0965952547 2014-10-15 Completion Testing No Violations On the day of my inspection Mercer Well Service was drilling out the plugs on the well for flowback operations. No violations noted.
1389363934 2014-01-09 Surface Facility Construction No Violations Received a call from Matt Potter with Gulfport. The plan to mobilize the conductor rig on Monday.
1400303287 2014-05-17 Casing String Witnessed No Violations Gulfport Energy is setting surface pipe and Sting-ray is cementing casing in place. There is 110of 26conductor pipe. There is 357 of 17.5 of open hole and 320 of 13.375 at 94lb/ft. They ran centrilizers at every joint. With a total of 9. There was a 13.375 float shoe and float collar ran. Sting ray ran 434 sacks of cement at Claas A cement @3%CaCl and .25lb/sack celloflake. There was good circulation with 10 barrel of returns.
1400510777 2014-05-19 Casing String Witnessed No Violations On the day of my inspection Patterson 480 had drilled the 12.25 open hole section. The 9.625 casing was ran and cemented. See casing ticket for details. No violations noted.
1413698381 2018-07-31 Production Wells No Violations Status check inspection found that there are 4 wells present and in operation on the well pad. The pad surface and lease road is of stone construction and in good repair. The pad entrance is gated and locked with signage posted at the end of the lease road next to Ohio State Route 148 and at the gated point of the well pad along with identification on each well head, the production tanks are also hazard labeled. Master emergency shut down well control devices are situated at the entry point of the well pad as well as at the production fluid storage area and at the well head and gas processing areas. The well head is equipped with a lower A section and upper B section, riser pipes with gauges installed are present on both sections to monitor annular pressure. The pressure on the 13.375 (A) section which is the pressure between the 13.375 and 9.625 diameter casing strings is 90 pounds per square inch (psi) The pressure on the 9.625 well head (B) section which is the pressure between the 9.625 and 5.5 diameter casing strings is 600 psi. The 5.5 diameter casing pressure is 800 psi. The well head assembly is equipped with a tubing head assembly with an emergency shut down device installed in the sales line downstream from the manual master valves. The sales line pressure is 600 psi. The company has installed a methanol injection system on the well head. Downstream from the well head assembly is the production equipment consisting of a gas processing unit with 500 psi pressure on the sales line at the unit. Immediately on the left as you enter the well pad is the production fluid tank farm consisting of (4) steel construction 400 barrel capacity tanks situated inside a polymer lined steel containment dike area. The well pad is protected by an earthen berm and there are fire extinguishers situated at all critical points on the well pad.
1439650490 2015-08-14 Preliminary Restoration No Violations On the day of my inspection the well appeared to have all equipment to be considered capable of production. The tank battery contained four 400 barrel production fluid storage tanks. The site was evaluated to confirm that proper vegetation and erosion controls had been established and maintained. This site has passed the preliminary restoration.
1475244081 2016-09-28 Production Wells No Violations On the day of my inspection the well appeared to have all equipment to be considered capable of production. The tank battery contained four 400 barrel production fluid storage tanks. The site appeared to be in satisfactory condition.
1931930816 2017-10-20 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains four 400-barrel production fluid holding tanks. Rice Energy appears to be providing midstream services due to signage on the location. Production records for this well are current to 2017 within RBDMS.
