0177971005 2018-05-07 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing with a plunger lift system, valves were open at the time of my inspection. Annular pressure monitoring was in place for the 9.625 inch diameter casing gauge read 0 pounds per square inch, the 13.375 inch diameter casing gauge read 60 pounds per square inch. Production equipment included a separator, Gas Processing unit, line heater, gas scrubber and 4- 400 barrel purge tanks shared with 3401320705, 3401320712, 3401320711. Identification tags were present and production was up to date.
0711415243 2019-03-11 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with the area disturbed by the well pad construction restored with vegetation established and no erosion noted. The well pad does not have a perimeter berm. The well pad and lease road surface are stone construction and in good repair. The production equipment is all on the well pad and contained in a separate fenced in enclosure. There is a total of (4) producing wells on the pad. All wells are equipped with 2 riser pipes attached to the A&B sections with gauges present for annular pressure monitoring. The wells are equipped with artificial lift production trees. The production equipment consists of 4 gas processing units along with a production fluid facility that consists of (4) steel tanks situated inside a polymer lined steel containment dike area. Proper well identification is posted, there is a landowner gate across the lease road with a digital entry box 4377# gets you through the gate
1290764795 2015-04-13 Production Wells No Violations Inspection conducted as a follow up to complaint # 4995. I received a call from Doug Bennett regarding a call alleging an impacted stream. During my visit, I walked the pad and outer limits of disturbance and found no presences of a release. I did not find any connection between this location and the complaint.
1568144862 2016-06-28 Production Wells No Violations This well is currently equipped with the proper equipment to be considered capable of production. During the inspection the well site appeared to be in satisfactory condition. No violations were noted.
2029950568 2013-10-24 Drill / Deepen / Reopen No Violations On the day of my inspection I was accompanied by Dept. of Commerce electrical inspector and Brad Sellers. We conducted an electrical inspection and are currently waiting on response from Commerce.
