0050469014 2019-02-06 Preliminary Restoration No Violations Status check inspection found the well site meets ORC 1509.072. The lease road and well pad surface are stone construction and in good repair. The well pad perimeter berm is protected by an earthen berm. 4 wells are present on the pad along with one gas processing unit and a tank battery with (2) 400 barrel tanks situated inside a polymer lined steel containment dike area. Proper identification is posted at the well head and lease road entrance point.
1354157426 2018-06-12 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing. Annular pressure monitoring was in place for the intermediate casing gauge read 1350 pounds per square inch, the surface casing gauge read 500 pounds per square inch. Production equipment included a Separator, Gas Processing unit and 2- 400 barrel purge tanks shared with 34013206720000, 34013206770000, 34013206960000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
