1423350833 2016-04-28 Production Wells Violations Noted On the day of my inspection I was following up on Chiefs Order 2014-138. The well was equipped with a pumping unit, tubing, and rods. The valves at the well were in the closed position. The tank battery consisted of a 100 barrel production fluid holding tank and a mechanical separator. Production records have not been submitted since 2004 and the well is unable to produce in its current state. Photos were taken on the date of this inspection. The well owner/operator shall submit a written plugging schedule to Inspector Andrew Corder at 5880 Memory Road Zanesville, Ohio 43701 within 30 days, per OAC 1501:9-11-OS(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

There was no motor present on the pumping unit. The production facility included a charting system, vertical separator, 100-barrel production fluid holding tank, and a 50-barrel fluid holding tank. The valves in the production area were in the closed position. No domestic production noted. Outside of the production area there was a 40-barrel fluid holding tank. No identification tags noted. Production records current to 2004 but no production filed after 2004.

2056538479 2018-10-15 Production Wells No Violations Compliance Notice # -1423350833 follow up inspection. On this day I traveled to the end of Anderson Run and made contact with the current landowner Nancy Boan who indicated that the roadway identified as Anderson Run had been vacated past the entry point of their Driveway and their was no longer access to the well site across their property. She did however grant me access to park on her property and walk across their property to inspect the well. The old power service for the well could be seen from their driveway on the North East side of the property. The well is directly past the power poles and across Anderson Run on a bench above the run slightly to the North East. There is a old 2 load line that runs from the tank battery in the direction of Anderson Run. Both the well head and tank battery is situated inside a fenced in enclosure with dense vegetation present around both the well head and tank battery. The well is equipped with 8.625 diameter surface casing with no lines attached to the surface casing well head assembly. There is a 4.5 diameter production casing string and well head present at surface along with a 1.900 diameter production tubing string with a tubing head stuffing box attached to the 1.900 tubing string with pumping rods inserted. Attached to the casing and tubing well heads are 2 diameter flow lines with check valves and manual valves in line. Both the tubing and casing flow line valves are closed and both lines are connected into a single steel production flow line that runs in the direction of the near by production facility. The well is set up to be produced by the means of a pumping unit with the drive motor missing and the power service disconnected. On location near the well head is a production facility consisting of (2) steel production tanks one being 100 barrels capacity and one 50 barrel capacity, the water mark on the 100 barrel tank showed approximately 12 or 12 barrels and the 50 barrel tank was checked and had approximately
