0777859165 2018-11-01 Production Wells No Violations Status check inspection found the well equipped with 4.5 diameter production casing and 1.900 diameter production tubing with both casing and tubing well heads present. Attached to the casing well head is a 2 diameter production flow line with a valve present in line and in the open position. Attached to the 1.900 diameter tubing is a tubing well head with a 2 diameter flow line present with a valve present in the closed position. There is a polish rod liner inserted into the tubing head with pumping rods present in the production tubing. The well is being produced by the means of a pumping unit driven by an electrical motor. Also present on the way back to the well head is a production facility that is shared between 2 wells in the area. The facility is equipped with 2 mechanical separators and 2 gas sales meters with 7 day sales chart in the meters. A single 100 barrel steel tank is present along with (2) plastic 2,100 gallon tanks. There is also a gas compressor and cooling manifold present in the storage area. All equipment noted in the storage area is situated inside an earthen dike area with well identification noted.
0943595837 2018-10-18 Production Wells Violations Noted On the day of my inspection accompanied by ODNR Supervisor Mike Moreland, I witnessed production equipment was shared with 34013205990000 and included a 100-barrel tank, and two 37-barrel plastic tanks, vertical separator, and 2-inch meter run with a .250-inch orifice and drip, and a small compressing unit and cooling manifold enclosed in an earthen dike. At the time of my inspection one of the 37-barrel plastic brine tanks had a 1-inch diameter plastic drain line through the earthen dike. I will contact Equity resources that the drain line needed to be removed. The tank valve was open, and the tank was empty at the time of my inspection. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, 1.5-inch diameter tubing and rods with a Churchill pumping unit, and an electric motor. When we arrived on location Andrew Ulrich of Amtek Oil was currently on location and cleaning up around the production equipment, after speaking with Andrew he informed us that he had been pumping the well for the past two days and they intended on purchasing the wells from Equity Oil and Gas Fund. After further investigation we discovered that he was currently pumping the production water through the 1-inch diameter valve on the pumping Tee directly onto the ground, there was a puddle approximately 10 feet in diameter near the well head. A field test was performed using Quantab titration test strips and revealed chlorides to be approximately 542 parts per million. We informed Andrew that he was not permitted to pump the water onto the ground, and he closed the valve and ran it to the production facility. Identification tags were not present, and production was reported as 0 since 2014. Equity resources will be contacted an informed of the issues that were encountered.

On the day of my inspection I witnessed production area was shared with 34013205990000 and included a 100-barrel tank, and two 37-barrel plastic tanks, vertical separator, and 2 2-inch diameter meter runs with a .250-inch orifice and drip, and a small compressing unit and cooling manifold enclosed in an earthen dike, it appeared that the well was using one vertical separator and one 37 barrel plastic tank as storage for production fluid. At the time of my inspection one of the 37-barrel plastic brine tanks had a 1-inch diameter plastic drain line through the earthen dike. The tank valve was closed, and the tank was empty at the time of my inspection. The well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, 1.5-inch diameter tubing and rods with a Churchill pumping unit, and an electric motor. All valves were closed at the time of my inspection, and new identification had been posted in the production area. At the time of my inspection the well was no longer causing pollution and contamination and the compliance issue had been resolved.

The production casing is 4.5-inches in diameter. There was a natural gas motor present on the pumping unit. There was an electric motor present on the pumping unit. The motor had belts that were attached from the motor to the pumping unit. The production area included one 100-barrel production fluid holding tank, two 37-barrel plastic tanks, a vertical mechanical separator, and a charting system. Zero production reported for 2016.
