0197807388 | 2013-03-18 | Orphan | No Violations | Visited site with Orphan well Inspector Cindy VanDyke. |
0360994821 | 2015-10-15 | Final Restoration | No Violations | My inspection found that the well location has had final grade and, per the landowners request, only the non-farmed portion has been seeded with rye grass, which is starting to grow. The landowner (Yanity) has signed a waiver stating that the restoration is satisfactory and nothing further needs done. I will check on the vegetative growth progress next week. |
0816869178 | 2015-06-29 | Plug / Plug Back | No Violations | Beginning today at 120, they ream down the hole and are at 150+ at 11 AM. Mark, the driller, notices that his pipe is spinning more slowly than usual and believes his hydraulic pump is wearing out. He will bring in a power pack unit tomorrow which will allow him a normal RPM rate again. There is a discussion as to other options that might increase the drilling speed but no viable solution is agreed upon. They spend the day reaming the hole and are at 162 when they shut down to go home. |
1013994264 | 2015-07-06 | Plug / Plug Back | No Violations | Today, the crew pulled the 7 casing along with the 3.5 casing out of the hole. The driller had to cut the 7 vertically and peel it off the 3.5 except for one 30 section that came out together. They found Cal-Seal inside the 7 from 50 down to 81. The 7 totaled 81 and was beveled out slightly, possibly made to sit on the seat in the hole . We also found 2 3/8 tubing inside the 7 and on the outside of the 3.5 casing from 58 to the total of 81. This was probably what was causing the 3.5 casing to stick in the 7. The contractor next beveled out a collar to 6.5 to go on the bottom of the overshot tool. He lowered the overshot tool in the hole and tried to grasp the remaining 2 3/8 tubing at 135 but was unsuccessful. We will try again tomorrow. |
1035307485 | 2012-12-28 | Orphan | No Violations | No casing visible at surface. Well is leaking oil, water, and a small amount of gas. Paraffin has formed a circular mound on the ground surface with the casing beneath the paraffin, fluids are leaking from this mound. Debris, including wood chunks and two pieces of 1 1/2 steel pipe has been stuffed into the mound by someone in the past. There is a contaminated area roughly 3x5 feet around the mound. The well is located in the flood plane of the Hocking River and is less than 100 feet from the river and less than 30 feet from a drainage ditch that runs into the river. Pictures taken. |
1060263456 | 2015-07-13 | Plug / Plug Back | No Violations | The crew prepared for cementing by running 4.5 casing to 155. The Huffman-Bowers cementing crew arrived at 10 AM. With fresh water in the tank, the cement crew mixed 82 sacks of 14.8#/gallon Class A cement and pumped it down the 4.5 casing. The well circulated and the cement stayed near the surface both inside the 8 5/8 casing and inside the 4.5 casing. The rig crew then pulled 3 joints, leaving the last 2 joints (64) in the hole to act as surface casing. The cement may need to be topped off tomorrow. A load of pit water was taken to the disposal well and the remaining pit cuttings were consolidated with cement. The crew spent the rest of the day taking equipment off location and should restore the site tomorrow. Time includes 6 hrs. clerical |
1084945254 | 2015-06-16 | Administrative Inspection | No Violations | With the rig back over the hole, the contractor, ties onto the 2 3/8 tubing and pulls to see if it is free in the hole. The tubing had no give but moves around freely at the surface inside the 7. As they did not yet have any fresh water on site, they only worked 3-4 hours this day and will come out in the morning to try to flush the tubing and evaluate what might be plugging it. I did not go out to the well this day. |
1129219503 | 2015-07-01 | Orphan | No Violations | The 3 1/2 tubing was hung in the hole. The company will attempt to unseat and pull the 7 casing Monday morning. |
1188564354 | 2014-10-20 | Orphan | No Violations | Review of location with potential bidder. |
1254914065 | 2015-06-10 | Orphan | No Violations | The diverter was removed and the fluids pumped out of the hole. There appears to be a metal obstruction roughly 10-15 feet down. |
1328161672 | 2015-06-24 | Plug / Plug Back | No Violations | At 6:30 AM today, the crew went in the hole with the repaired 2 3/8 overshot tool to pull more tubing but are unsuccessful - the pipe appears to be cemented in tight. Next the rig crew mixed mud, put together the changeover and float subs and went into the hole with the 6.25 milling bit, knowing they will be drilling steel. The milling bit fell in the hole to 53 and they milled the 2 3/8 tubing to a depth 60, which is the depth that the 4.5 bit had already drilled to. The milling progress slows down considerably. Late afternoon, the rig crew decides to build another bit, using a 3.5 collar with tungsten carbide built up on the bottom edge of the collar. At 5 PM and still at 64, they go in the hole with the 3.5 bit and 3.5 drill steel and found it to drill better. At a depth of 66, left for the day. |
1343548721 | 2013-05-13 | Orphan | No Violations | I spent day working on title work for this well. |
1346006255 | 2015-06-17 | Plug / Plug Back | No Violations | With a load of fresh water in the tank, the crew used the mud pump with no pump pressure to try to circulate the tubing and found it circulated out the 7. Next, with the tubing and casing full, the crew ran water down the tubing and measured 1.66 barrels in the tank before the well circulated out the 7 , indicating that the well would be @ 415 deep, based on volume calculations and if there were no holes in the pipe. Mark and the crew will bring out the oil saver, swab tools and another pump to do tests to determine if there is a hole in the tubing or the water is going out the bottom of the tubing. The crew worked only the morning hours and got rained out. |
1367861301 | 2013-03-18 | Orphan | No Violations | My inspection this day with Josh Garland to begin process for entering the well into orphan well program. This well is leaking oil and may qualify for the Coast Guard plugging program. Oily area around the well is @ 5 x 3 and hole is bubbling gas and leaking oil. Well is located about 40 from a ditch that drains into the Hocking River - once contamination hits drainage ditch, it is only 20 until it reaches the river. The well appears to be an open hole with no casing and has 1.5 steel tubing sticking in the hole to mark the location. Well card from the Survey is the only record available and there is no indication that the well has been plugged. SEM is 216, pictures taken. |
1369237817 | 2013-03-18 | Orphan | No Violations | My inspection this day with Josh Garland is to see if this well will qualify for the Orphan program. This well is leaking water, oil and gas with an area about 3 x 5 around the well soaked with oil and paraffin. The well is 40 from a drainage ditch, then another 20 where it can enter into the Hocking River. Heavy rains will put this well under water. I found 1.5 tubing sticking up in the well bore to mark it that can easily be removed - no sign of any casing in the hole. This well may have been plugged in the past and has started leaking but I am more inclined to believe it was never plugged - have found no well records to indicate plugged. A title search at the courthouse indicates that these were shallow, Cow Run wells at @ 380 and drilled in the 1920s or earlier. Two other wells on the property are also in question as to whether they have been plugged. The small (5 bbl?) tank at the road should also be removed if not in use. The tank marks the entrance for access to the wells. Pictures taken, SEM is 216, and believe this well should qualify for Coast Guard emergency plugging. |
1374066531 | 2013-07-17 | Orphan | No Violations | Pictures taken of well and contaminated area. |
1433383674 | 2014-12-16 | Orphan | No Violations | Met with possible Orphan bid contractor at the well to discus plugging operations. |
1433933461 | 2015-06-09 | Plug / Plug Back | No Violations | Today was spent bringing equipment to the site and setting up the rig to begin work on the well tomorrow. Drill pipe and mud pump were brought and the water tank for cementing was set up. The rig was positioned over the hole. Tomorrow will go in the hole with the bit and see if we can find bottom. |
1433964196 | 2015-06-10 | Plug / Plug Back | No Violations | Today, after repairing the mud pump then mixing the mud in the pit, the rig crew went in the 7 diameter casing with a drill stem and 4 3/4 bit to clean out the hole. They got a good show of oil at 8-10.6, then hit something very hard at 10.6. They drilled on it for 1.5 hours and made no progress. At this depth, they do not have enough weight to put much pressure on the bit to drill. They pumped the hole dry and were able to see, what appeared to be 4 diameter casing and 1 diameter tubing in the hole, bent sharply over. The decision was made to excavate down around the casing, below the obstruction at 10.5, and cut off the 8.62 and 7 casings in order to expose and remove the obstruction. This will be done tomorrow. |
1434114724 | 2015-06-11 | Plug / Plug Back | No Violations | Today the smaller track hoe dug out around the 8 5/8 casing to widen and deepen the hole to a depth of 12 so that the 8 5/8 casing could be cut off below the obstruction inside of this casing. The 7 casing had been unscrewed at a depth of 22 leaving only the 8 5/8 to be cut off. |
1434369061 | 2015-06-12 | Plug / Plug Back | No Violations | Today was spent preparing the 8 5/8 diameter casing to be cut off at 12 below ground level to expose the obstruction that is inside this casing, at a depth of 10.5. The Contractor excavated around the casing, brought lumber and built a box around the hole which was used as shoring to keep the hole from caving. They also welded a 1 collar and nipple onto the outside of the 8 5/8 casing, below the obstruction and installed a valve. They then cut a hole thru the casing so, if required, they could drain the oil and gas through valve. Crew left location at 4:30 PM. Contractor to cut off the casing Monday morning. |
1434467914 | 2015-06-15 | Orphan | No Violations | The company has dug down 10 feet and cut the casing apart. A roller wheel from belt track was removed. The company is welding a new section of pipe to bring the well back up to ground level. |
1435075533 | 2015-06-19 | Plug / Plug Back | No Violations | The 4.5 casing needed to drill over the 2 tubing arrived and was unloaded. It will be used to drill/clean out the annular space between the 7 diameter casing and the 2 diameter tubing, in hopes of freeing up the tubing so it can be removed. The rig starts drilling at 1 PM with the 4.5 pipe and a notched out collar on the bottom to cut debris in the hole. At 2 PM they are 50 down and beginning to drill out cement. The driller continues down over the 2 tubing until they hit something very hard at 60. They also notice that their pit full of drill water is lowering quickly. They added 30 barrels of water and within a half hour, the pit is down to the same level as before adding the water. At 4 PM, we will have to get more water to drill with and decide to shut down for the weekend. The driller says that after the rig and mud pump were shut down, the well flowed back all the 30+ barrels of fluid that it had taken when drilling. The driller, plans to get more water and mix up the mud still in the pit from the last well. He believes that the 7 casing is in good shape but that the 8 5/8 casing is not completely seated in the hole and that is causing the well to wash out on the outside of this casing. |
1435240378 | 2015-06-22 | Plug / Plug Back | No Violations | Beginning this day at 60, the driller mixes mud to prevent further lost circulation from Friday, and drills making only 2 in 2 hours. Mark, the driller, comes out of the hole with pipe and bit and constructs a new collar with teeth and notching out areas to adapt the bit to work better. This bit works only slightly better making a total of 2. They continue to drill to 2:30 PM when they decide to try a milling bit and go to WVA to get the and change-over subs. I finish up the day with clerical. |
1435338008 | 2015-06-25 | Plug / Plug Back | No Violations | Today we began drilling at a depth of 66 and ended at 140, drilling the entire day with the 3.5 fabricated bit made from the 3.5 collar. The zone at 109 where we had found a hole in the tubing drilled quickly, due to the void in the cement on the backside of the tubing. The rig then went back to drilling metal and cement. |
1435683833 | 2015-06-26 | Plug / Plug Back | No Violations | Beginning at a depth of 140 today, drilling continues to be slow . At 162, because of the very slow drilling, the rig crew decides to pull their pipe and bit to inspect it for wear. They find it is worn and out of gauge and Mark welds more tungsten carbide on the bottom as well as the sides of the bit which should keep it in gauge. They go back in the hole and find they are reaming beginning at 120, due to the gauge change of the bit. They shut down for the day at 120. |
1435770647 | 2015-07-01 | Plug / Plug Back | No Violations | Today, we tried to remove the 3.5 drill pipe stuck in the hole at 150. Yesterday, the rig crew was drilling down between the 2 3/8 tubing and the 7 intermediate casing when the pipe suddenly got tight and would not move. We circulated the hole and found we had good circulation but no cuttings were coming to the surface. The driller wanted to drive the joint that was half way out of the hole, back down to where they could make a connection. He built an attachment that he used as a driver and attached it onto the bottom of his jars. Using this, he was able to pound the casing back down the hole. Next he had to re-enforce the ground around the well as it was compacting and he could not pull hard on the casing to remove it. He moved the rig off and filled the area around the well with more dirt, then placed more mats on top of the dirt. He then moved the rig back over the hole and pulled again, finding that the 7 casing was coming out of the hole with the 3.5 casing. We will not work again until 7/6/15, when we will pull the 7 casing with the 3.5 tubing. |
1436191955 | 2015-06-30 | Plug / Plug Back | No Violations | Started drilling at a depth of 170 At 2 PM the drilling had slowed considerably and at a depth of 178 the contractor decides to pull and examine the bit. He is able to pull out one joint and when he starts to pull the second joint it pulls very tight and will not come out. The front of the rig is coming off the ground and the ground is settling around the hole, but the pipe refuses to move. The contractor will have to move the rig off the hole and fill in with more dirt around the hole. He will also need to add more mats to stabilize the rig so he can pull harder on the pipe. We are at a depth of 150 when we end for the day. |
1436373561 | 2015-07-07 | Plug / Plug Back | No Violations | This morning, the rig crew went in again with the overshot tool but could not get over the top of the 2 3/8 found at 135. There is 55 of tubing unaccounted for. The contractor goes to the shop to build 2 new bits and goes in the hole with the 6 1/8 bit on 4.5, designed to go inside the 7 but over the 2 3/8 tubing. At 103, they start reaming the hole out, removing the Cal-Seal inside the 7 and drill/ream to 145. They made one trip in with the overshot tool on 3.5 casing but are unsuccessful in getting it. Tomorrow, they will continue to try and pull the 2 3/8 out. My total time includes 3 hours of clerical. |
1436455222 | 2015-07-08 | Plug / Plug Back | No Violations | Today, we tried again with the overshot tool to retrieve the 2 3/8 tubing, grabbing on to it but losing it again. On the second try, we get last joint of tubing out which consists of a 15 mud anchor, indicating the bottom of the tubing would be at 150. We continue to drill with the 6 1/8 bit on the 4.5 casing to determine if this is the bottom of the hole or just a depth they have plugged back to. The rig crew slowly drill to 166, checking the samples for fire clay which would indicate the well was plugged back or drilling new formation, indicating we are drilling new hole. When we stop to collect samples, the pipe gets stuck in the hole and the driller cannot move it up or down. We will come back tomorrow and continue to work on it. |
1436829947 | 2015-07-09 | Plug / Plug Back | No Violations | Today, the crew rigged up the tools used prior to drive down the 4.5 casing. They also switched out one 32 joint for a 10 pup joint of 4.5 casing. The casing was lowered and circulation was achieved in just a few minutes. The crew cleaned the hole and brought all the casing out. Jet Wireline arrived at 2:30 PM to run a CCL, gamma ray, and bond log of the hole. The log showed 8 5/8 casing ran to 47.5 with no cement behind it (it was driven in) and the sand and gravel ran to 42. The top of the Peaker Ss. which contained oil in the area was 86. It was decided to run 4.5 casing to 64 to protect the ground water. Preparations were made to cement 7/13/15. |
1437047106 | 2015-07-15 | Plug / Plug Back | No Violations | Today, the crew had dug out around the casing and was ready to cut it off when I arrived. They cut the 8 5/8 and the 4.5 casing off 40 below ground level because it was a plowed field. The crew topped it off with less than a half bag of cement and attached the steel plate with permit number over the hole. They filled in the area around the hole and emptied the pit containing solidified cuttings, placing it in a dump truck. The entire area was rough graded as well as the access road. Because of the very muddy conditions, they may have to come back but today sewed grass seed on the area not part of the field. I will check it after conditions dry out to see if more work is needed. |
1438630205 | 2015-08-03 | Plug / Plug Back | No Violations | Today I conducted a Final Restoration inspection but found the location cannot be passed as complete. No grass is growing and it looks like the area has not had rain since we plugged the well. I will wait longer until it rains for a few days, then re-inspect. |
1446760159 | 2015-11-02 | Final Restoration | No Violations | This location has passed the final restoration inspection. Mr. Yanity has signed a Landowner Waiver, stating that he is satisfied with the reclamation and doesnt need grass growing on the property as the ground will be put in crops. |
1451405472 | 2015-10-21 | Final Restoration | No Violations | On inspection this day, I found all debris removed, the land leveled and restored to its original condition and the grass beginning to establish on the creek bank. Mr. Yanity had requested the crop ground not be seeded with grass and signed the Landowner Waiver stating he is satisfied with the restoration. Final Restoration is passed. |
1525778509 | 2015-06-23 | Plug / Plug Back | No Violations | The driller screws into the 2 3/8 diameter tubing that is 11 down and inside the 7 diameter casing. and pulls out 1 joint of tubing. He goes in with the overshot tool and pulls another joint out but breaks part of the fishing tool. The tubing has now been removed down to a depth of 53, which is the depth at which he encountered cement when drilling with the 4.5 fabricated bit which is acting as an overshot tool. They prepare to go in with the milling bit and find out they do not have the right connections/subs for the milling bit to go onto the 4.5 drill pipe. to make it work. They make another trip to WVA to get the parts. They leave the rig @ 2:15 PM to go to WVA. |
1693198974 | 2015-06-17 | Orphan | No Violations | 1.6 bbl fluid pumped down tubing to establish circulation and estimate depth. The annulus between the tubing and the production casing was full of fluid, the estimated depth of the point of circulation is 415 feet. |
1759764484 | 2015-06-18 | Plug / Plug Back | No Violations | The driller brought the 2 3/8 diameter swab out to the rig and ran in the tubing to determine if it was clear enough to cement through. He found that the swab stopped at 109 and would go no deeper. He then rigged up the pump and, with fluid entering above the swab, pumped the swab tools down and found that fluid was going no further than 109, indicating a hole in the tubing. This also affects the estimate of @415 from yesterday, making it invalid. The crew worked only part of the day. Tomorrow, they will have 4.5 casing on location to wash down over the tubing. They hope to determine if there is a packer on the 2 3/8 tubing, how deep it is and to clear the backside of the tubing for removal. The tubing may have to be fished out piece by piece. The crew only worked part of the day. |
2038657929 | 2015-06-02 | Orphan | No Violations | Stopped by site to verify pit construction and take photos. |
2066850630 | 2013-06-10 | Orphan | No Violations | Undate title search, review. |
2070465927 | 2015-07-08 | Orphan | No Violations | The mud anchor was removed at 150 feet. The company was drilling out the bottom when I arrived and the cuttings were examined and found to contain steel scale from the inside of the surface casing. The company shut down and the drill bit got hung in the hole. |
2078967074 | 2015-06-25 | Orphan | No Violations | An obstruction was encountered inside the tubing and tools were only able to be ran to 109 feet. The tubing was parted at 50 feet. Now the company is drilling out cement at 170 feet with a fabricated overshot/drill bit tool on a 3.5 tubing with a power swivel. This improvised tool and drill stem combination allows the company to drill out the cement from in-between the tubing and the 7 casing. |
