There was no evidence of discharge of fluids outside of containment. Am-Tek was notified to connect the 1 inch plastic sump line to the tank.
0077493021 2018-05-30 Salt Water Injection Well No Violations I received a call from inspector Matt Fulk. Matt found a brine kill caused by a flow that runs from the tanks to the well. The kill area was approximately 20 feet by 30 feet. I tested the soil for chlorides and found the highest reading to be 400 PPM. Approximately 4 inch of soil was removed and placed in a line pit. This material will be hauled out and properly disposed of. The leak in the flow line was located. AM-TEK oil has the well shut down until they decide how to repair the leak in the flow line.

The dike and pump house are not free of water but the level is coming down. I will recheck first of next week to see if the dike is free of water.

There was some oily substances on top of the ground. The scalded area was approximately 25 x 40. I notified the Andrew Ulrich who sent the pumper to meet me at the tank battery facility. I showed the pumper where the scald was and talked about the course of action to remediate the location. Buddy the pumper stated he would bring his small 4 wheel drive Kubota tractor with a bucket and hoe attachment to remove the affected soil.

Water in dike field tested and found to have no Chloride content. Injection tubing at well was on vacuum, however I was unable to accurately test the annular pressure. Upon opening the 3/4 needle valve on the annulus I found grease squirting from the open valve. Mechanical integrity was proven, but immeasurable. I estimated the annular pressure to be around 10 psi for reporting purposes.

I traveled to an area with cell reception and called Andrew Ulrich the owner/operator of the well and explained I would need the dike emptied of water. He explained that the sump pump went out and that he had given a new pump to his well tender and that he would call him and have him get on it right away. I called acting UIC inspector Kip Thompson and explained the situation and let him know of my intentions.

Trees had grown up on and inside dike. Trees shall be removed from dike. The loading area has a crack that allows ground water to seep up through and into the dike. The cracks in the loading area shall be repaired. The dike shall be emptied of all water. The dike shall be kept free and clear of water at all times.

Pump pressure down at facility read 70# and pump had been running less than an hour, pressure up at well still on vacuum, and annulus pressure at 0. Will do 15 min. annulus test next quarter when drier - well up on top of big hill - operator said takes 70# to pump water up the hill, pump only pumps 2 bbl/hr. Floor of pumphouse has skim of oil on it, water has been pumped out but when rains, tends to flood the pumphouse. Told them I would like to see more dike next time they have dozer in, floor cleaner. Good IDs found at pump and well. Road to well rough but still passable. Inspection time includes driving time.

Inspected the unload pad, containment dike, tanks, injection pump and containment sump pump. The sump pump 1 inch black plastic line has been placed inside the containment dike. The 1 inch line is not connected to a tank. The injection operations are shut in, due to the injection line needing to be replaced.
