1436715017 2015-07-12 Production Wells Violations Noted I was notified of a pinhole leak in a two inch steel flow line associated with the Tri County #8 well by Dave Krippin of Stonebridge operating. Roughly 1 bbl of fluids escaped from the line, and traveled down an embankment 30 feet to a ditch along Co Rd 48 in Bern twp., Athens Ohio. GPS was taken at the site; 39.36617 -81.93742. Contaminated materials then moved 50-60 feet through the ditch. Five men were on site at the clean up. 12 bags of debris and contaminated soils were removed from a 50 x 20 area around the line and down the embankment. A large sump was hand dug across the flow along the length of the embankment to collect run off of contaminated fluids. Two small dams and a larger syphon dam were constructed in the ditch line to contain the flow of oil and fluids as the embankment was washed with fresh water. An 80 bbl vac truck was on site to removed fluids from the dams and sumps. Booms and pads were in place to absorb contaminated materials. Any faulty equipment shall be repaired or replaced. All contaminated materials shall be removed from the site and disposed of properly. Any affected areas shall have topsoil replaced and steps shall be taken, including seeding and mulching, in order to establish vegetative cover.
