The dike has been removed and there is no contamination at the site, the area has been graded over. Submitted to orphan program.
0429065828 2017-02-13 Orphan No Violations Orphan well status inspection: Upon inspection, it is observed that this well is still idle, incapable of production, and is likely orphan. Previous inspection reports indicate that the well used to have a pumping unit tank battery. The pumping unit and tank battery have since been removed from the well location. The well now consists of 7 surface casing (buried), 3.5 production casing, and 1.5 production tubing with rods. There are no flow lines attached to the well at any connection and the well is shut-in. Previous inspection reports indicate the production tank leaked brine into a pit, and there is still a vegetation scald area (8 x 10) downhill of where the tank battery sat. Additionally, there is a separate hole in the ground (approx. 8 dia.) 50 east-southeast of the well head, which is full of water to surface. After probing the hole with a stick, it is observed to be bridged/plugged 12 down. The updated Site Evaluation Matrix score for the well is 133. Photos were taken.

Well equipped with pumping unit. 100 Barrel steel tank, separator, and a meter box make up the tank battery. There is roughly 3 inches of standing water in the lower half of the dike.
0978106246 2013-01-07 Production Wells No Violations Asked to check on complaint status by previous inspector Cindy VanDyke. The well is equipped with a pumping unit but the power supply is not connected. The production casing was closed and the valve on the tubing was open. The 100 Barrel production tank was placed less than 100 feet from the well and contained some fluids. The dike surrounding the tank had 3-4 of water in it at the time. The water was tested and found to have a 575 ppm Chloride content. The dike wall had been cut some time in the past to allow water to flow from the dike, there was no evidence of significant contamination outside of the dike. A second tank by the road was empty and there was no water or contamination in the dike. There was significant erosion along the lease road in the form of a rut roughly one foot deep.
1291651478 2010-12-06 Production Wells No Violations Called Johns number and talked to a granddaughter - Johns son is out of town and didnt have a phone number for him. She said the estate had been settled for 6 months or more, that she knew that he had 1 well in Athens Co. and nothing more about it than that. I believe well was not mentioned in documents as son was not wanting to plug it - as well shows as Historic Owner, I anticipate turning it over to the Orphan well program to be plugged. Mr. Kelly may want to reconsider what is done with well and I will talk to him about further plans. Supervisor had asked me to check on probate status.
1402425806 2014-06-10 Orphan No Violations Valve on production closed and valve on tubing is open. Pumping unit overgrown with vegetation and well appears incapable of production.
1421835400 2013-07-08 Orphan No Violations My inspection this day is to collect information for submittal of the well into the I/O well program. Mr. Tom Kelly, landowner, said he would be willing to plug the well with a Land Owner Grant and we will proceed with that approach. The well was completed in 10/1978 to the 2nd Berea Sand at 1760 and Mr. Kelly believed they fraced the well and got too much water to produce it. On location, I found 1 100 bbl tank with separator, meter run and meter, drip, and a pumping unit on the well. The well was too overgrown to see if there was a motor on the unit. There is a pit behind the tank full of fresh water and numerous steel and plastic lines heading down to the road. Down at the road, (Mill School Rd) at Mr. Kellys drive sets another 100 bbl tank with dike, used to hold oil for the hauler to pick up. The well is equipped with 3.5 production casing and 7 surface casing, rods & tubing. The lease road is overgrown but is still used to get to another well on the property. Pictures taken, SEM is 86.
