0153695203 2015-09-01 Production Wells No Violations I left the site to follow up on other notices and left Trish Cartmell to witness the removal of the contaminated sand and gravel lense. When I returned the roll off box was gone and the area had all of the top soil replaced. I confirmed with Trish the company had removed all of the contaminated sand and gravel lense and taken it to a land fill. They then replaced the topsoil. The area still needs to be graded and have vegetative cover established.

1438616871 2015-08-02 Production Wells Violations Noted Got called by Kip Thompson about a spill in Athens County. Mark Hines contacted him about it. He is cleaning up the job for the owner. I will be heading out there asap. It is 8:30 Sunday night.

I arrived on the site shortly after 10pm. Mark Hines has 2 workers with him doing the cleanup. The estimated loss from measuring the tank is 23bbl. I talked with the owner of the well and he thinks they got it contained at its furthest point. Barry Bowers arrived shortly after I did with his water truck. The tank is being emptied at this time. They are planning on sucking the pools of oil out of the creek. They have diapers and booms spread throughout the contaminated area to soak up what they can. I will send an update in the morning.

1438725182 2015-08-03 Production Wells No Violations 8:00 am: I arrived on location to relieve Inspector Justin Smith. Upon arrival, Mark Hines and Mark Abdella were using two track hoes to finish up soil excavation from underneath the tank pad location. All contaminated soil has been stockpiled on pit liner material and covered until it can be hauled away for disposal. A hard boom and underflow dam was installed in the creek approximately 1/3 mile downstream from the spill origin, no fluid was being released from the underflow dam as of yet, but likely will if a rain event occurs. Booms and oil absorbent pads are in the creek in several spots beyond the underflow dam to catch any product that gets past. Huffman-Bowers has two vac-trucks on location and are recovering oil from two temporary pooling/collection areas in the creek. SW Region ER Manager Trisha Cartmell arrived on location at 9:30 am and remained there to direct cleanup upon my departure. As I was leaving, one vac-truck was preparing to dump a load of water in the creek to flush free product to a common collection point where the other vac-truck could recover it. I will stop by this location again on my way home to check cleanup status.

3:30 pm: Back on location to check progress of cleanup. Mark Abdella of PDQ O&G was using a track hoe to remove oily round straw bales from the creek and placing them in a roll-off box. Huffman-

1438878198 2015-08-05 Production Wells No Violations The clean up at the site is continuing. The contaminated soils are being removed and a sump has been dug and filled with water to flush the oil out of the sand lense in the soil.
1441158940 2015-09-01 Production Wells No Violations Met Mark Abdella and Trish Cartmell. The contaminated stockpile was not on site and it was possible to see where the company had excavated down to the contaminated sand and gravel lense over the weekend. The hard boom in the creek seems to have been topped by a small amount of contaminate and has moved down stream a bit but is being held by the soft booms in place. There still appears to be some contamination leaching from the soil along the bank. We spoke with Aaron Wolfe with OEPA over the phone to get an idea of his questions and concerns about the clean up in the creek itself. He was fine with allowing the small amount of contaminate to leach through the bank and into the creek as long as the booms were being maintained. The contaminated sand and gravel lense under the stock pile area will be excavated and taken to an EPA approved landfill.
