1284554767 2010-09-14 Production Wells No Violations Inspection this day found still no IDs, tank now has gravel dike. Talked to County, owner of the well, and explained what they needed to do to get bonded, insured and registered as operator, what was needed for IDs. Supervisor Stonerock with me.
1286833418 2011-03-24 Administrative Inspection No Violations Follow up to inspection # 1298918399. I spoke with Mike Canterbury of Athens Co. engineers office on March 24, 2011. Mr. Canterbury informed me the clean up had been completed and the engineers office was in the legal process of having the well moved to its name from Dennis Chapman. Upon inspection that day I noticed the clean up was in fact complete. Mr. Canterbury informed me they would need a few weeks to correct ownership issues. I granted a one month extension to original NOV to have ownership issues resolved.
1298918399 2011-02-25 Production Wells Violations Noted No ID was found on the wellhead or production tank, wellhead had sprayed oil on an area roughly ten square feet onto ground, No production records have been filed in last two years. 1501:9-9-05 Producing operations A(10). All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tanks as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. If multiple wells are being produced into the common tanks each well-head shall be identified as to owner, permit number where available, well number and lease name. Remedy: Well and tank should be equipped with proper identification. All oily materials shall be soaked up, removed, and disposed of in a proper manner. Affected ground should be tilled, reseeded, and mulched after oily materials have been removed.
1318944985 2011-10-18 Production Wells No Violations Spoke with receptionist at Athens County Engineers office in regards to ownership of well. Previous owner and Athens County had just recently signed a form 7 and sent it to be reviewed. I left my contact number with Mike Canterbury to have him call me with the details.
