0004013419 2018-10-11 Orphan No Violations Berry Well Service was drilling out clay during my inspection. Berry Well Service will drill out to approximately 370 feet and install approximately 350 feet of 7 inch casing. Time of inspection: 8:25 AM.
0117759332 2018-10-12 Orphan No Violations I witnessed the cementing of the 7 inch casing on the Frank Homer #1 well. Berry Well Service installed 355 feet of 7 inch casing. Cemenco was used for cementing services. Cemenco achieved circulation and pumped 160 sacks of Class A cement. Good cement returns were observed. Time of inspection: 10:25 AM.
0250845863 2018-10-18 Orphan No Violations I arrived on location about 8:35 am. The rig crew drilling out and making joints. They made about 5 joints while I was on location. I left my card with Brian and explained that I would be in the area if they needed anything. They were below the Berea Sandstone formation.
0326397711 2018-10-22 Plug / Plug Back No Violations I arrived on location about 8 am 5 rig crew and cementers (Cemenco) arrived about 8:40 am. they placed bottom plug 400 feet. Pipe was dragging hard coming once out of cement pulled another joint to ensure was out of cement and hooked up to mud pump and circulated on the string. Then pulled out to top of lime in bigger hole went back in and placed the lime plug. Appalachian Wire Line services arrived, ran three logs and tagged cement.
0402553721 2018-10-24 Plug / Plug Back No Violations I arrived on location around 7:50 am Brian the rig operator was already there and opened the tubing and the circulating pipe. The well appeared to be dead with no pressure and the water level right there. We will cement out of the hole this morning once the cementers arrive. Placed two plugs in the hole one from 440 to 240 and one from 188 to 0. Donnie Strange called me about 1:30 and said they were ready to cut the pipe off and place the monument on top and cover up the wellhead. I went and witnessed the task.

Mr. Walls was agreeable to the proposed items in the plans for the project and requested that the 2 steel vent line be placed in the fenceline along the western border of his property.

This well is bid out to be plugged in the Athens #1 Plugging Project for fiscal year 2018. The landowner, Mr. David Walls, was notified that the Contractor plans on plugging the well during the month of July 2018.

1479308859 2016-11-15 Orphan No Violations Orphan well investigation, pertaining to complaint #5466: RBDMS records indicate that this well was plugged with prepared clay in 1937 and was found to be leaking gas by a previous landowner in 1952. The landowner subsequently attempted to use the well for domestic gas, but was unsuccessful. Upon inspection, it is observed that the well is idle, incapable of production, and is likely orphan. The well consists of 10.875 outside diameter casing rising 30 above ground level. There is a square steel plate welded to the top of the casing. The casing is tapped with a 1 diameter threaded bung which leads to two 1 diameter 90 degree elbows, a 1 diameter pipe nipple, and a 1 diameter ball valve which is in the closed position. Coming out of the ground 6 east of the casing is a 0.5 diameter vent pipe with a 0.5 diameter ball valve in the closed position. There are no flow lines attached to the well casing. There well is leaking gas out of a hole in the casing which is just above ground level. Gas odor was noticed up to 30 from the well and the PhD6 gas detector read 17% LEL and 0.55 ppm CH4 at 10 away from the well. The readings subsided quickly as wind blew through the area. The well is situated 105 west of Elm Rock Road in a fenced-in area and is 50 northwest of an occupied house on the adjoining property, 82 south of the landowners garage, and less than 130 south of the landowners house. The area surrounding the well is a flat residential lawn. Tree lims overhanging the well are from trees on the adjoining property and additional permission will be needed from that landowner to remove limbs from them. There are overhead as well as buried utility lines running along Elm Rock Road which would need to be crossed under/over to access the well. There is no existing lease road for the well. A meeting will need to be set up with the landowner to discuss potential ingress/egress options. The Site Evaluation Matrix score is 223. Photographs were taken of the location.
1482146869 2017-07-05 Administrative Inspection No Violations Mr. Roger Smith called at 10:12 am to discuss a 0.5 diameter steel gas line that was located on this orphan well site. Mr. Smith stated that the line was cut off near the fence line separating his property from David and Vicki Walls property when municipal water supply lines were run to his home and Dows Rollarena. Mr. Smith also stated that the line was completely empty when it was cut off and it was left open-ended under the surface after it was cut. This information has been passed along to Kris Roser of the Engineering Section for site-planning and restoration consideration.
1488292047 2017-02-13 Orphan No Violations On location this day to evaluate access route options and gather utility information for this orphan well location. Upon evaluating various access points, it is concluded that the best option for accessing the well is through the landowners backyard, which would be accessed by temporarily removing a section of the northwest corner of the landowners fencing. The northwest corner of the property will be accessed via the driveway for Dows Rollarena (pending landowner permission). The landowner, Vikki Walls, states that they are on a public water system (Le-Ax Water), and they have a septic system. The septic tank was located at GPS: 39.42732, -82.19189, but Ms. Walls was not aware of the exact location of the leach bed. There are no natural gas lines on the property. No water wells were observed on the Walls property, however there are 3 water wells within the 500 area of review. The area surrounding the well and along the proposed access route does not drain well and water lays in large puddles after rain events. Mud mats will be necessary along the access route if the well is plugged during a wet time of year. Photos taken of proposed access route.
1497882737 2017-06-13 Orphan No Violations On location with DOGRM Engineer Kris Roser to evaluate ingress/egress and site engineering considerations. Temporary removal of the landowners fencing and tree removal/trimming options were discussed. The project site is very close to the property boundary, and a large maple tree which overhangs the well from the adjacent property will need to be trimmed. A right of entry form was left in the door jamb of the adjacent property owner, Mr. Matt Warren.
1498585006 2017-06-27 Orphan No Violations Orphan well field work: On location to investigate a 0.5 diameter steel gas line which was located by the DOGRM Survey Section. According to adjacent landowner, Mrs. Helen Smith, the line was once hooked up to a small wall-mounted heater inside Dows Rollarena when Mr. Dow McCune owned the building. The line is no longer in use, but may still enter the building through its foundation. Mrs. Smith also states that the line was abandoned before 1957 when she lived in Mr. McCunes mobile home that used to be on the property. Mrs. Smith also states that the well was once hooked up to the water heater in Mr. McCunes mobile home, but it did not produce enough gas to be a reliable source. Mr. Roger Smith is out of town this day, but will be available next week to show where the line enters the building. Mrs. Smith was informed that the Division will need to access the line to flush and cap it during wellsite reclamation. NOTE: In addition to gathering gas line information, the Landowner Right of Entry form has been received this day from Mr. Matt Warren, the adjacent landowner to the south of the project location.
1540473361 2018-10-16 Orphan No Violations I arrived on location about 1:20 pm. They were drilling out the old hole. They got on top of something about 860. The BOP rubber was leaking and decided to just circulate on the hole and change the rubber before drilling into the Berea.

2095409924 2019-04-26 Orphan No Violations I received a request from Regional Manager Mr. Dan Goins to respond to a complaint from the landowner where this well was plugged. I met with the landowner and Mr. Bill Mitton with Knox Energy. Mr. Mitton agreed to remove straw that was placed over disbursed areas in the landowners lawn. Once the straw is removed, topsoil will be spread and seed will be applied. The straw will be replaced on the topsoil.
