1496850434 2016-06-30 Orphan No Violations My inspection today was conducted to determine the status of Orphan Well complaint number 01-015 received in 2001. Inspector Kohl responded to the initial complaint and found a well equipped with 7 inch casing inside of 10 inch casing in a wooded area being cleared for future development. Inspector Kohls report stated the well was equipped with a 7 inch swage reduced to 2 inches installed on 7 inch casing and that a two inch open valve on the swage was emitting a steady stream of gas. There is no production or domestic equipment evident for this well. There are not Division records for this well. The above permit number was assigned for tracking purposes. The Ohio Fuel Gas map shows an Oriskany well on this property but not in this location. Older, unpermitted Oriskany, Shale and Clinton wells are shown in the surrounding area on the Ohio Fuel Gas map. My inspection today showed the well is located in a mowed grassed area approximately 8 feet south of a 10 foot wide graveled drive and approximately 105 feet west of Ninevah Road. An open plastic barrel has been placed over the casing and the 10 inch casing that was visible in 2001 is no longer visible. A 7.3 inch O.D. collar with a swage reduced to 2.5 inches is installed on the 6.7 inch casing. A 2.4 inch O.D. fitting with a closed valve is installed on the swage. My Sperian gas monitor did not show measurable levels of gas near the well. There is not a detectable odor of gas near the well at this time. Rocks have been placed around the barrel and a pond is now present approximately 18 feet south of the well. A small camping trailer is approximately 140 feet northwest of the well in a mowed clearing. The clearing where the well, camper and pond are located is surrounded by woods. A wood fence and gate prohibit vehicle entrance on to the property from Ninevah Road. An active railroad is approximately 250 feet south of the well. 1843434405 2017-05-12 Orphan No Violations My inspection today was conducted to determine the status of Orphan Well complaint number 01-015 received in 2001. My June 2016 inspection did not detect any gas leaks or a noticeable gas odor in the area of the well, see the June 2016 report. Todays inspection showed the valve on the well is still shut. There is a noticeable gas odor near the well. There is not a visual leak at the well. My Sperian gas monitor detected a 40% LEL/0 H2S near the swage but water poured over the well did not show the location of the gas leak. There are no fluids leaking from the well at this time.
