0254902875 2018-05-15 Drill / Deepen / Reopen No Violations The lease road, pad was rocked and accessible. The wellhead was surrounded with jersey barriers. The valves on the 9.625 X 7 annular and 7 X 4.5 annular spaces were opened and free from any flow. There were 14-500 barrel frac tanks in preparation for the frac scheduled for 6/4/2018. The pits were closed and rough graded. There were two (2) mud pumps, one (1) substructure and a rack of drill pipe left on location. These items will be removed once drilling activities start. Final restoration was schedule to take place once the frac and completion activities are done.
0320529404 2018-04-23 Casing String Witnessed No Violations I was on location with Shannon Mullins for a training session and observing the running and cementing of the 4.5 production casing. Topics reviewed included volumetric calculation of cement tops, zonal isolation and cement weight. U S Energy Drilling Rig # 9 drilled a 6.25 borehole from 2978 KB to 5680 MD. The rig ran 133 joints of 4.5, 11.6, LS, 8rd, ST&C casing. A float shoe, 24 bow spring centralizers, latch baffle and wiper plug were included in the casing hardware. Petroset Cementing Services circulated 84 barrels of cement slurry ( 15.2 verified ) from 5658MD to 1300 TVD ( calculated ). The plug was landed at 1000 psi and increased to 1500 psi for five (5) minutes without any pressure decline. After the pressure was bled off .5 barrel flowed back and stopped. The float held. No violations identified during this inspection.
0641107981 2018-04-23 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 4.5 inch production casing. US Energy OH, LLC drilled a 6 1/4 inch bore hole to a depth of 5680 feet. US Energy ran 138 joints of 4.5 inch casing to 5654 feet, with 24 centralizers. Petroset Cementing Services (PCS) connected to the casing and established circulation and conditioned the bore hole with 200 lbs of gel. PCS mixed a cement slurry consisting of 400 sacks of Class A cement blended with 2% CaCl. The cement slurry was displaced to 5648 feet GL with 87.5 barrels of fresh water. Upon displacement of the plug, pressure was increased to 1500 psi and held for 5 minutes.
0899006205 2018-04-02 Drill / Deepen / Reopen No Violations I was on site to witness a downhole camera inspection of the intermediate 7 inch casing. Appalachian Well Service was on location to run a downhole camera to inspect the 7 inch casing. Prior to the camera inspection, a bond log was conducted on the 7 inch intermediate casing, and came back showing complete cement coverage around the casing. Pressure would still not hold when applied to the 7 inch casing, so the downhole camera inspection was conducted. During the camera inspection, two locations on the 7 inch casing showed splits. US Energy made the decision to repair the areas of split casing, prior to completing the drilling of the well.
0940677194 2018-03-30 Drill / Deepen / Reopen No Violations I was on site to witness the Blow Out Preventor (BOP) test on the intermediate casing string. Prior to conducting this test, the required pressure after immediately landing the plug, was not able to be held. Several attempts to obtain pressure on the landed plug were unsuccessful. At the time of this inspection, U S Energy Rig # 9 closed the annular Blow Out Preventor (BOP) against the six inch drill collar and attempted to pressure up the seven inch casing. No pressure was able to be obtained and held against the seven inch casing. I advised US Energy, LLC to conduct a bond log test to ensure cement coverage and casing integrity.
1032068831 2018-04-05 Casing String Witnessed No Violations I was on the Kuhar # 2-H location to witness the lower portion of the seven (7) inch intermediate casing undergo a pressure test. A split was located in the intermediate casing at 1010 DF. U S Energy lowered a Parmco packer, on 3.5 drill pipe, to 1035 and engaged the packer. This isolated the intermediate casing from 1036 to 2968 ( casing shoe depth ) in order to confirm the integrity of the casing below the split. This section of casing was pressured up to 620 psi for twenty minutes without any pressure decline and this section was deemed to have its integrity. U S Energy was conferring as to remedial actions to be taken. No further violations identified.
1102864248 2018-03-29 Drill / Deepen / Reopen No Violations I was on location to witness the cementing of the intermediate casing. US Energy OH, LLC drilled a 8 3/4 inch bore hole to a depth of 3003 feet. US Energy ran 70 joints of 7 inch, casing to 2978 feet with fifteen centralizers. Petroset Cementing Services (PCS) connected to the casing and established circulation, conditioned the bore hole. PCS mixed a cement slurry consisting of 400 sacks of Class A cement blended with 3% CaCl. The cement slurry was displaced to 2978 feet GL with 120.5 barrels of fresh water. Upon displacement of the plug, pressure increased, then dropped immediately and no pressure could be sustained. I advised US Energy to conduct a blow out preventor (BOP) test to ensure casing integrity. If the the BOP test failed, a bond log would have to be conducted.
1411881353 2018-04-08 Casing String Witnessed No Violations I was on location to witness the pressure test to confirm the integrity of the seven (7) intermediate casing after repairs were performed. U S Energy Rig # 9 closed the annular Blow Out Preventor (BOP) against the six (6) inch drill collar and pressured the seven (7) inch casing to 720 psi. This pressure was held for 30 minutes without any pressure decrease. The test indicated that the intermediate casings integrity was re-established. I informed U S Energy that it was permissible to resume drilling ahead.
1521115091 2018-03-13 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the conductor casing. US Energy OH, LLC drilled a 17.5 inch bore hole to a depth of 68 feet on a fresh water fluid system. US Energy ran three joints of 13.375 inch, 36 lb/ft conductor pipe to 63 feet with one centralizer. Petroset Cementing Services (PCS) connected to the casing and established circulation, conditioned the bore hole with 40 lbs. of cottonseed hulls. PCS mixed a cement slurry consisting of 105 sacks of Class A cement blended with 3% CaCl. The cement slurry was displaced to 55 feet GL with 8 barrels of fresh water. I observed approximately 9 barrels of cement slurry returned to surface.
1521222243 2018-03-15 Drill / Deepen / Reopen No Violations I was location to witness the placement and cementing of the 9.625 inch surface casing. US Energy OH, LLC drilled a 12.25 inch bore hole from 68 feet to 341 feet while circulating a fresh water based drilling fluid. US Energy placed 7 joints of 9.625 inch 26 lb/ft, casing and four bow spring centralizers, and a cement basket to a depth of 326 feet GL. Petroset Cementing Services (PCS) connected to the casing and conditioned the hole with 200 lbs of gel and established circulation. PCS mixed a cement slurry consisting of 150 sacks of Class A cement blended with 3% CaCl and 40 lbs of cottonseed hulls and circulated it back to surface. Cement slurry verified by scale at 15.3 ppg. The cement slurry was displaced to 326 feet GL with 26 barrels of fresh water. I observed approximately 17 barrels of cement slurry back to surface.

1775133452 2018-05-22 Completion Testing No Violations I was on location to check on frac preparations for the Kuhar # 2-H. Moore Well Services and Production Service Corp were on location for toe preparation. The frac sleeve was set at 3780MD. After repeated attempts, with pressures up to 6000 psi, the frac sleeve failed to open. The toe preparation will be completed with tubing conveyed perforation prior to stimulation.
