0021181732 2017-10-27 Drill / Deepen / Reopen No Violations U S Energy Rig # 9 was drilling ahead on saltwater based drilling fluid at 4628 MD. Approximarely 900 remained to be drilled to total depth. I inspected the pits, cellar and mud pump area and did not identify any violations.
0128179374 2017-10-19 Drill / Deepen / Reopen No Violations The drilling pad was serviced by a graveled lease road. The drilling pits had adequate pit volume and were fenced. I checked the cellar and the various casings and did not observe any gas migration to surface. The air compressors and booster area was free of leaks and contamination during this inspection. U S Energy Rig # 9 was removing the drill pipe from the wellbore in preparation to attach the directional bottom hole assembly. No violations identified during this inspection.
0566403178 2017-10-23 Drill / Deepen / Reopen No Violations U S Energy Rig # 9 was running into the wellbore to drill an additional 30 to allow Navigator Wireline Survey to run a Gyro logging tool downhole for correlation purposes. The pits had adequate volume during this inspection. An inspection of the cellar and casings stubs did not indicate any gas migration. No violation found during this inspection.
0654008494 2017-10-30 Drill / Deepen / Reopen No Violations U S Energy Drilling Rig # 9, after circulating the borehole for two hours, was removing the drill string from the borehole after reaching total depth at 5455 MD. The pits had adequate room and I did not identify any leaks or contamination surrounding the mud pump area. The wellbore was static. I counted 135 joints of 4.5 11.6 lbs. / ft. casing on location. No violations encountered during this inspection.

The tool pusher ( Cub) does not anticipate spudding well before 10/9/2017. The pits allocated to the Mate # 1H well were lined and stabile. Reviewed casing program and drilling permits conditions again. No violations identified during this inspection.
1507049428 2017-10-03 Drill / Deepen / Reopen No Violations U S Energy Rig 9 was moving on the Mate Unit # 1H location. The lease road and drilling pad was graveled with adequate access. The drilling pad measured approximately 200 X 200 with the pits on the north side of location. There were four (4) pits, two (2) for fresh water and two (2) for brine. I reviewed the permit conditions ( Shallow Surface Casing Permit and Horizontal Drilling Permit conditions ), Inspector notification points and casing and cementing design programs with Cub ( U S Energy Tool pusher ). I did not identify any violations during this inspect.
1526393012 2018-05-15 Preliminary Restoration No Violations The access road was graveled to the pad. The pads dimensions were 180 by 200 and contained two wells, Mate Unit # 1-H & # 2-H ( 34-007-2-4564 ). The drilling pits were filled and rough graded. The Mate Unit # 1-H was completed as an electric pumping well. The well was operating during my inspection. The tank battery was constructed on the north side of the pad. The tank battery consisted of three (3) 210 barrel stock tanks and one Gas Production Unit ( GPU). The load lines were plugged and locked. The I. D. information was written on the tank and pumping unit. An earthen dike surrounded the tank battery. The dike was seeded and covered with straw, but was not growing. The perimeter of the pad was rough graded but not seeded. I spoke with Cary LeShock ( U S Energy ) who said, the landowner did not want U S Energy to seed the disturbed area for fear of weeds getting into his hay field and he would seed it when dry enough . A three (3) inch gas sales line was awaiting connection to a sales point. The well was started pumping on 5/9/2018. A re-inspection is warranted to assess compliance with the Preliminary Restoration parameters.
2085704068 2017-10-10 Casing String Witnessed No Violations U S Energy Rig # 9 spudded the Mate Unit # 1H on 10/10/2017 @ 2:30 pm and drilled a 17.5 borehole to 70 KB on a fresh water gel system. A conductor casing string consisting of 63.4 of 13.375, 48 lbs. / ft. ST&C was installed in the borehole with a bow spring centralizer at 43 KB. The borehole was conditioned by circulation for approximately 1.5 hours. Petroset Cementing Services ( PCS) connected to the conductor casing and circulated 40 lbs. of cottonseed hulls ahead of the cement slurry. The cement slurry consisted of 100 sacks of Class A cement blended with 3% CaCl2. Mix water parameters were 62 F, pH was 7. The cement slurry weight was verified, by scale, at 15.2 ppg. I witnessed approximately 10 barrels of cement slurry returns to surface. I did not see any sign of gas migration in the cement slurry after pumping ceased. Plug was down at 11:20 pm with 100 psi left on the casing.. The drilling pits had adequate volume and freeboard. No violations identified during this inspection.
2097078823 2017-10-31 Casing String Witnessed No Violations U S Energy Rig # 9 drilled to a total depth of 5455 MD and ran 129 joints of 4.5 11.6 lb./ft., WT-80 casing to 5423. Casing hardware included 17 bow spring centralizers, latch down baffle and a five wiper plug, toe sleeve and a 4.5 float shoe. Petroset Cementing Services conditioned the borehole with 10 barrels of gel prior to cementing with 400 sacks of 50/50 Pozmix. Displacement of the cement slurry was achieved by a five wiper plug and 84 barrels of water. The plug landed at 900 psi and the pressure was increased to 1400 psi for the Green Cement Test. After five (5) minutes the pressure was released and the float held. No violations identified during this inspection.
