0101005725 2017-06-22 Casing String Witnessed No Violations I was on location to witness the cementing of the 4.5 production casing. U S Energy reached a total depth of 5929 MD on the Gaag # 1-H. After lowering 141 joints of 4.5, 11.6# casing with 21 solid body centralizers to 5907 the well was circulated until the fluid was clear. Petroset Cementing Services cemented the casing with 400 sacks of 50/50 Pozmix, 2% gel, 10 % salt. Mix water temp. was 75 F and pH was 7. Four samples of cement slurry were weighed and ranged from 13.8 ppg to 14.0 ppg. The slurry was displaced with 92 barrels of water and plug. The plug landed at 700 psi and was increased to 1200 psi for five (5) minutes. I did not witness any pressure decline. Once release back the casing flowed back .5 barrel and stopped. The float held. No violations.
0291471777 2017-08-28 Preliminary Restoration No Violations I was on the Gaag pad to observe the pit closure. The pit system consisted of four (4) pits, a working and cuttings pit for each well. Petrowater Services ( UIC # 52 ) was the hauler of the pit fluids. Some of the fluid was transported to the Bates pad ( Brookfield Twp., Trumbull Cty. ) for use in the stimulation of those wells. The remainder fluid was disposed at the Shalelogic facility in New Lyme Twp., Ashtabula Cty. U S Energy was the contractor for the closing of the pits. After the pits were emptied the cuttings were equalized among the four (4) pits allowing the cuttings to be encapsulated at approximately eight (8) feet below grade. No violations identified during this inspection.
0942931411 2017-09-18 Production Wells No Violations Moore Well Service was removing the 2.375 tubing to clean the 4.5 casing free of sand and debris. Once the casing is clear Moore will replace the tubing and run sucker rods to complete as a pumping well.
1039585609 2017-05-11 Drill / Deepen / Reopen No Violations I was on site to witness the pressure test on the BOP. The company pressured up on the drill stem annulus to 360 psi then shut-in the annulus. They held the pressure for 15 minutes with no loss of pressure. The test passed.

Sure Shot Wireline was the contractor for perforating guns and plugs. The well was scheduled for 17 stages ( 100 length ), five (5) clusters of 10 shots each per stage. No violations identified during this inspection.

The access road and well pad was rocked and accessible. The disturbed area where the drilling pits previously existed was planted with oats and was beginning to grow. The 210 barrel stock tanks were properly identified. No violations identified during this inspection.

They established circulation, they pumped a 30 barrel gel flush follow by 400 sacks of 50/50 poz and displaced to 5862 with 91 barrels of water. Plug landed with 1800 psi. released to tank plug held
1754776497 2017-07-05 Production Wells No Violations The Gaag pad was being configured for completion activities. Eleven (11) 500 barrel frac tanks were spotted on location. Ten (10) tanks were for frac water and one (1) tank to be used as a hydration tank. The pits has sufficient free board since the pit fluid will be the frac fluid. Plan Forward was to begin completion activities the week of 7/10/2017. No violations identified during this inspection.
