0053118986 2018-10-18 Production Wells No Violations I inspected this well pursuant to Chiefs Order #2016-300 issued for lack of insurance. At the time of this inspection, the well was equipped with a Concoil plunger lift assembly and not in production. The valve on the casing head was closed. There was an audible gas leak from the 4.5-inch casing threads. The tank battery was comprised of one 100 BBL tank and a mechanical separator. The tank battery was diked and had barely legible identification. There remained no active insurance for the owner.

1043590133 2018-02-27 Production Wells No Violations I, along with Supervisor Tom Hill, inspected this well pursuant to Chiefs Order #2016-300 issued for lack of insurance. At the time of this inspection, we noted the following: a wellhead with concoil-style mechanical plunger lift system. The primary gate valve at the production casing head was closed. There was a detectable leakage of gas around the back-side of the 8.625-inch surface casing. The wellhead and battery were in an open clearing surrounding by dense grasslands and shrubbery. The interior portion of the battery were overgrown with grasses. The production elements appeared to be in poor condition and not well maintained. There were no signs of on-going leakage or newer leaks or spills. The access road to the wellhead and battery was a shared-use, gravel-paved road that was in good condition. The well was not in active production. There remained no active insurance for the owner.

I then took photos of dead fish and pond area . I then contacted Wildlife to report a fish kill . I spoke with Bud Miller at Portage Lakes office , He advised me they do not regulate private ponds .
