0355657153 2018-10-18 Production Wells No Violations I conducted an inspection on this well pursuant to Chiefs Order #2016-300 issued for lack of insurance. At the time of this inspection, the well was equipped with a Concoil plunger lift assembly and not in production. The valve on the production casing head was closed. There was an audible leak of gas at the back-side of the 8.625-inch surface casing. The tank battery was comprised of one 100 BBL production tank, and mechanical separator. The interior portion of the battery were overgrown with grasses. There was some area within the diked battery that were stained from previouos spills or releases of oil had occurred. No leaks were observed at the time of this inspection. The tank battery was diked. No legible identification was present at the well or tank battery. The lease access was stable. There remained no active insurance for the owner.

1486062242 2017-10-31 Production Wells No Violations I inspected this well pursuant to Chiefs Order #2016-300 issued for lack of insurance. The well and associated battery are located on an otherwise undeveloped parcel. At the time of this inspection, I noted the following: a wellhead with concoil-style mechanical plunger lift system. The primary gate valve at the production casing head was closed. I did not note any signs of leakage or impact to the surrounding area. The wellhead and battery were in an open clearing surrounding by dense grasslands and shrubbery. The associated tank battery was comprised of one 100-barrel production tank, and horizontal separator. The interior portion of the battery were overgrown with grasses. There was some areas were historic spills or releases of oil had occurred as evidenced by dark, staining and heavily degraded oil deposits within the battery. The production elements appeared to be in poor condition and not well maintained. There was no signs of on-going leakage or newer leaks or spills. The access road to the wellhead and battery was a shared-use, gravel-paved road that was in good condition. The well was not in active production.
1519839300 2018-02-27 Production Wells No Violations I, along with Supervisor Tom Hill, inspected this well pursuant to Chiefs Order #2016-300 issued for lack of insurance. At the time of this inspection, we noted the following: a wellhead with concoil-style mechanical plunger lift system. The primary gate valve at the production casing head was closed. There was a detectable leakage of gas at the 4.5-inch casing threads. There was also an area of stressed vegetation and partially exposed pit liner at the location of the former pits. The wellhead and battery were in an open clearing surrounding by dense grasslands and shrubbery. The associated tank battery was comprised of one 100-barrel production tank, and horizontal separator. The interior portion of the battery were overgrown with grasses. There was some areas where historic spills or releases of oil had occurred as evidenced by dark, staining and heavily degraded oil deposits within the battery. The production elements appeared to be in poor condition and not well maintained. There was no signs of on-going leakage or newer leaks or spills. The access road to the wellhead and battery was a shared-use, gravel-paved road that was in passable condition. The well was not in active production. There remained no active insurance for the owner.
