0085254407 2017-07-12 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice inspection 557985989 and complaint 5667. At the time of this inspection, the well was not in production and the valves on the flow line was closed. No updated production records have been filed for this well and the last reported production was in 2011. There was an audible leak around the stuffing box. The stuffing box was covered in oil and old oil staining around the well casing was observed. The tank battery still had a tank valve that was dripping brine and there was water surrounding the 210 BBL production tank. I conducted a high range Quantab test of the water and it maxed out the Quantab. The tank battery was diked and the water was contained within the diked tank battery. No signage has been added to the well or tank battery locations. I contacted Charles Moseo about the leaking tank valve and to repair the leak at the stuffing box on the pumpjack. I also requested that Mr. Moseo remove the water from the tank battery location.
0335817680 2019-01-09 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1931418690 and complaint 5667. At the time of this inspection, the well was not in production and equipped with a pumping unit. There has been no effort to mitigate the leak at the stuffing box or to install identification at the well location. At the shared tank battery, the leak at the tank valve was still slowly dripping brine from the 210 BBL production tank. No identification has been installed at the tank battery location as requested in previous inspection reports.
0557985989 2015-07-06 Production Wells No Violations Follow-up: Complaint # 5010, Jake Ward. My inspection found the oil lying on the ground at the well location has not been removed, also there is still fluid dripping from the valve at the base of the production tank. I attempted to contact Charles Moisio by telephone to receive no response.
1098052041 2015-05-13 Production Wells No Violations Follow-up, Complaint# 5010, Jake Ward. My inspection of the leaking production tank found there is still fluid leaking past a plug in the valve at the base of the tank on the east side. There has been nothing else done to resolve the compliance notice.
1186345646 2018-10-24 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1931418690 and complaint 5667. At the time of this inspection, the well was not in production and equipped with a pumping unit. There was an audible leak around the stuffing box near the tubing packing. The stuffing box was covered in oil and old oil staining around the well casing was observed. There was a 50 BBL poly tank at the well location half filled with water. The shared tank battery still had a tank valve that was dripping brine at the 210 BBL production tank. The tank battery was diked and shared with well 2-1926. No signage has been added to the well or tank battery locations as requested in previous inspection reports.

I also requested that Mr. Moisio add signage at the well and tank battery locations.
1548138679 2018-02-14 Production Wells No Violations I inspected this well as a follow-up to Chiefs Order #2014-202 for failure to maintain liability insurance. At the time of this inspection, there was a pump jack system with closed flow-line connections. There was a two-tank battery with separator located at the lease road entry point (presumed to be shared with the Loretta Engle (#2-1926) located north along the same access road. There were no signs of active production. There was no identifying signage. There remained no active liability insurance on file for Plateau Energy Corp.
1931418690 2015-04-22 Production Wells Violations Noted Complaint# 5010, Jake Ward. ODNR Division of Oil & Gas records show this well has not been drilled. My inspection found the well is equipped with a pumping unit with all the equipment in tact to produce the well. There is a 1100 gallon plastic tank five feet north of pumping unit which was one quarter full of fluid, also there was another 1100 gallon plastic tank located ten feet east of pumping unit which was one half full of fluid. There was oil stained vegetation east of pumping unit in a 30X10 area. The oil has not flowed from area. There is no identification posted at the well location. This well shares a common tank battery with permit number 1926. The tank battery is equipped with one 210 bbl, one 110 bbl steel tanks and one separator. The 100 bbl tank is dripping fluid from the unloading valve at the base of the tank on the east side. I tested the fluid that had pooled under the valve with a 1176 quantab. The result of the test showed the chloride level was over the maximum limits of the quantab [6400 ppm ]. There is no identification posted at the tank battery area. The well is not in production at this time. The last reported production for this well was 54 bbls of oil, 865 mcf of gas and 141 bbls of brine in 2011. There is a unresolved Chiefs Order for this well for failure to maintain liability insurance issued on 7-14-2014. REMEDIAL WORK REQUIRED TO RESOLVE THE VIOLATIONS: REPAIR LEAKING TANK IMMEDIATELY, OBTAIN LIABILITY INSURANCE, UPON OBTAINING LIABILITY INSURANCE PUT WELL BACK INTO PRODUCTION, REMOVE OIL STAINED VEGETATION AND SOIL FROM WELL LOCATION, ESTABLISH IDENTIFICATION AT THE WELL AND TANK BATTERY LOCATIONS, SUBMIT PRODUCTION REPORTS FROM 2012 TO PRESENT, SUBMIT WELL COMPLETION REPORT FOR THE WELL. NOTE: I found no oil on or near the pond on the property. I spoke with Katie Brainard [ landowner ], she told me there was no problems with her water well. I contacted Charles Moisio [ current well owner ], I advised him of the violations and t
