0117813527 2018-09-14 Production Wells No Violations I conducted an inspection on this well as a follow-up to Chiefs Order #2014-202 for failure to maintain liability insurance. At the time of this inspection, the well was equipped with a pumping unit and being utilized by the landowner for domestic gas only. The valves on the commercial production side of the well were closed. The tank battery consisted of one 100 BBL tank and a mechanical separator. Valves on the separator were closed. The domestic line was plumbed into the separator and the valve on that line was open. The tank battery was diked but did not have any signage. No liability insurance had been obtained from Plateau Energy Corp. There have been no changes to the well location.
0451986283 2018-10-24 Production Wells Violations Noted I conducted an inspection on this well. At the time of this inspection, the well was equipped with a pumping unit and not in production. There was an open valve off the 4.5 inch casing with an active domestic line. Production for this well was last submitted in 2002. The diked tank battery consisted of one 100 BBL tank and a mechanical separator. Valves on the separator were closed. No chart or chart box was present in the separator or tank battery. The domestic line was plumbed into the separator and the valve on that line was open. No identification was present at the well or tank battery locations. Lease access was stable. The well owner shall submit a written plugging schedule to Inspector Shannon Mullins at ODNR Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days. The well owner/operator will be notified if the plugging schedule is approved or denied. In the interim, the owner must correct the identification sign.

At the time of this inspection, the well was being utilized by the landowner for domestic gas, but the well was not being produced commercially based on the closed flow-line connections. The was a pump jack unit and associated tank battery (comprised of one 100-barrel steel production tank with mechanical separator) located behind the referenced house. Elements of the system appeared to be in disrepair and the flow-line connections were closed with the exception of the domestic connection. There remained no active liability insurance on file for Plateau Energy Corp.

Valves on separator were shut. The domestic line was plumbed into the separator and the valve on that line was open. No identification was posted at the tank battery.
