1237371616 2017-10-03 Production Wells Violations Noted The lease road was accessed off of S R 46. The road was graveled to the tank battery and continued to the wellhead on a farm lane. The well was configured as a plunger lift well utilizing 1.5 tubing inside of 4.5 production casing. The well was surrounded by a broken down chain link fence and overgrown vegetation. The I D signage was present but not legible. The well was not cycling during my inspection. I did not identify any leaks or contamination at the wellhead. The tank battery was approximately 130 east of S R 46 and was partially surrounded with a worn down earthen dike. The tank battery equipment included one (1) - 210 barrel stock tank, one (1) - 100 barrel brine tank and one (1) horizontal mechanical separator ( 2408 ). The I D signage was not legible and requires replacement. The chart on the gas meter reflected the current production period. The floor of the tank pad around the 100 barrel brine tank was stained red and had salt crystals coating the floor. The 210 barrel stock tank had free and mobile oil leaking out of the heater tube hatch ( previously repaired ) on the north side. These occurences are violation of the OAC 1501:9-9-05 (8 , 9 & 10 ). Which states, in part, (8) All surface equipment must be pressure rated to withstand operating pressures to which it is subjected and (9) The dike shall be kept reasonably free of water and oil and Al producing leases shall be legibly identified in a conspicuous place . The Owner / Operator, in order to bring the McCarthy # 1 into compliance, shall remove both the 100 barrel and 210 barrel tanks, excavate the contaminated soil from the tank pad, dispose of the soil and tanks properly ( supply disposal manifests to DOGRM ), rebuild containment dike and place legible I D signage on the replacement tanks. The measures prescribed herein shall be completed and approved by the Divison of Oil and Gas Resource Management Inspector by 11/06/2017. Please contact Bob Roush at 330.690.8837.

1518795809 2018-02-13 Production Wells No Violations I conducted a Compliance Notice follow up inspection to assess if the remedies detailed in Compliance Notice # 1237371616 ( 10/6/2017 ) were completed. The original Compliance Notice detailed issues concerning a leaking 210 barrel stock and 100 barrel brine tank and failure to identify the well. My inspection on 2/13/2018 found the leaking tanks had been removed, contaminated tank pad material removed and identifying signage was posted. A single 100 barrel stock was installed with fresh gravel to serve as the tank pad. The earthen dike was reconstructed and legible identifying signage was posted on the tank. The previous violations of 1501:9-9-05(A)8, 1501:9-9-05(A)01 and 1501:9-9-05(A)10 have been corrected.
1856833540 2017-11-28 Production Wells No Violations I was contacted by Ron Hamen ( Petrox subcontractor ) on 11/24/2017 to inform me that the tank replacement and remediation as required in Compliance Notice # 1237371616 was scheduled to be begin today. My inspection indicated that an excavator was dropped off on location. No personnel were present. I called Mr. Hamen who informed me that activities were delayed a day due to water truck availability. The requirements of the Compliance Notice have not been met.
