0319698444 2015-05-12 Production Wells Violations Noted Complaint# Alan Wenger. My inspection of the well found the tank battery is equipped with one 100 barrel steel tank, one buried concrete tank and one separator. There is no identification posted at the tank battery area. Further inspection of the well site area I found several feet of plastic gas line piled near the well. There is also a trenching machine parked next to the well. There is rods protruding from inside the tubing in the well bore. There is no pump-jack unit for the well. There is a active domestic hook-up off of the production casing. I spoke with Jaimie Coe[ well owner ] and Levi Miller[ landowner ]. They both had conflicting stories to the actions from both parties to the activity at the well. REMEDIAL REQUIRED TO RESOLVE THIS COMPLIANCE NOTICE: POST IDENTIFICATION SIGN AT THE TANK BATTERY AREA..

0912310104 2017-11-02 Production Wells No Violations This inspection was conducted as a follow up inspection to assess compliance with Notice Of Violation # 1488817388. I found that thw Mead # 1 well was not capable of production, idle and not plugged. The well was completed as a pumping well with tubing and rods. The pumping unit was removed previously. The well was not connected to the flowline. The domestic gas meter read 145300. The tank battery consisted of one (1) 100 barrel stock tank, one (1) 2408 separator and one (10 in ground concrete brine vault. The 100 barrel tank bottom seam had caused oil staining on the ground . The concrete brine vault was severly deteriorated on the east side with the brine level ready to overflow the breach The I D signage was present and legible. The Mead #1 was incapable of production and the Notice of Violation ( # 1488817388 ) remains in effect.

1197224255 2018-10-01 Production Wells No Violations I conducted an inspection to assess the level of compliance with a Chiefs Order No. 2018-220 ( issued 8/6/2018 ). The Mead # 1 was not in production nor was it plugged. The wellhead was configured to operate as a pumping well. There were rods sticking up through the stuffing box. The corrosion on the polish rod indicated that there was any pumping movement recently. The well was not equipped with a pumping unit or motor to actuate the rods to pump. The flow line was disconnected from the tubing, rendering the well incapable of production. There was a domestic gas meter which read 1472 on the digital readout face. The tank battery consisted of one (1) 100 barrel stock tank, one (1) concrete in ground brine vault and one (1) horizontal separator. The 100 barrel stock had a faded I.D. sign posted and was leaking the tank contents from the bottom seam on the north side. The concrete in ground brine vault was deteriorated where the lid connected with the body of the vault. The fluid level was equal with this deterioration. This Mead #1 was not plugged as required in the Chiefs Order No. 2018-220 ( issued 8/6/2018 ) at this time of this inspection.
1428446151 2016-09-27 Production Wells No Violations Myself and Rick Anderson [ supervisor ], met with Levi Miller and two of his sons at his place of business to discuss his concerns of the well located on his property.
1488817388 2017-03-02 Production Wells Violations Noted My inspection of this well found this well was not in production. The last reported production for this well was in 1994. This well was equipped with one 100 barrel steel tank and one separator. The well had rods and tubing in the production casing. The pump-jack had been removed. This well was incapable of production at the time of my inspection. I took 3 photos of the well, 4 photos of the tank battery area, 2 photos of the domestic hook-up at the residence and one photo of the entrance to the tank battery. REQUIRED REMEDIAL WORK: Plug this well also James Coe shall submit a written plugging schedule to Jerry Kohl at the Ohio Department of Natural Resources Division of Oil & Gas Resources Management 3575 Forest Lake Dr. Suite 150 Uniontown, Ohio 44685-8116 within 30 days, per OAC 1501:9-11-05[ B ], and plug the well within 180 days. James Coe will be notified if the plugging schedule is approved or denied.
1491318757 2016-08-29 Production Wells No Violations My inspection today was conducted with District Supervisor Anderson and Inspector Perry to determine the status of this well. The current landowners, CVAS Holdings LLC., reported the well has been non-productive and they have not received a royalty for approximately 10 years and the well does not have enough pressure to supply gas for domestic use. They stated the operator removed the pressure regulator from their domestic line and the pump jack has not been on the well for approximately 20 years. Inspector Kohl will follow up on this complaint.

1497552235 2017-05-24 Production Wells No Violations I was on location to meet with Levi Miller to discuss the status of the well. At the time of my inspection the well remains idle. The reading on the domestic meter was 1428 Cubic Feet. The flow line from the well to the tank battery remains disconnected. Both ports on the tubing head are disconnected and shut off. The 8.625 inch casing annulus is tied into the 4.5 inch tubing head on the west side. The 4.5 inch tubing head valve is open and tied to the domestic supply. Gauges installed on the domestic supply read zero. The polish rod is completely corroded and pitted. The tank battery contains one 100 barrel production tank, a brine vault and a mechanical separator with a meter box and chart. The chart in the meter box showed no production. Two jersey barriers are in place at the end of the access road. No effort has been made to plug the well at this time.
1504105163 2017-07-28 Production Wells No Violations At the time of my inspection the well was idle. The flow line from the well to the tank battery was disconnected at the well. The polish rod showed no indication of being moved, it remanded heavily oxidized. The 8.625 inch casing is connected to the 4.5 inch casing by a flexible hose. Tubing valves were closed and not connected to any thing. The domestic meter reading was 143600. The chart in the chart box has not been changed. The vegetation around the tank battery and wellhead are being maintained by Aaron Miller. I meet with Aaron and Levi Miller during inspection. The well remains idle and in Non Compliance.
1507310520 2018-01-29 Production Wells No Violations On location to meet with the landowner Levi Miller and to check the status of the well. The well is configured for a pumping unit which is not present. The polish rod is oxidized. The stuffing box is dry. The flow line is disconnected from the well. The tubing valves are closed. The surface casing annulus is plumbed into the production casing. The production casing is only connected to a domestic supply system. The domestic meter read 146500 at the time of the inspection. The well was not in production at the time of this inspection. The concrete vault at the tank battery has a hole in it below the lid, east side of the vault center. Salt leaching was evident around the perimeter of the vault. The vault needs to be emptied, steam cleaned and removed from the battery. The well has not been plugged as required by the compliance notice issued 03/06/2017.
1533058035 2018-07-27 Production Wells No Violations On location to meet with the land owner Levi Miller and check the status of the well. The area around the well is overgrown with a mix of perennial plant species. The well is configured for a pumping unit, which is not installed on it. The polish rod is oxidized. The stuffing box is dry, oxidized and has a light growth of moss on it. The well is not in production. The well has not been plugged. Two calls were placed to James Coe, both went to voice mail, which was full. The Compliance Notice to plug the well that was issued 03/06/2017 has not been complied with. Well still needs to be plugged.
1550018932 2019-02-07 Production Wells No Violations Chiefs Order No. 2018-220 was issued 8/6/2018 ordering the well to be plugged. At the time of my inspection, the wellhead was configured to operate a pump jack unit. The polish rod was sticking out of the stuffing box in the air. The well was also equipped with a domestic hook up with a drip bottle and a meter. The domestic supply was shut off. The flow line was unhooked from the tubing. The tank battery consisted of one 100--220.

1796136165 2018-05-15 Production Wells No Violations I conducted a follow up inspection to assess the level of compliance with Notice of Violation # 14888173888 issued 3/6/2017. My inspection revealed that the Mead # 1 was not in production nor was it plugged. The wellhead had rods sticking out of the tubing without any pump jack to actuate them. The well was not capable of production. There was a domestic house connection with a meter. The meters numerical readout was at 1472. The 100 barrel stock tanks bottom seam was leaking oil and causing contamination. The buried concrete brine vault was deteriorated at the seam with its top. The brine level was ready to breach the tank. The I. D. signage was posted. The Mead # 1 was incapable of production and the Notice of Violation # 1488817388 ) remains in effect.
1854910044 2017-09-18 Production Wells No Violations My inspection of the Mead # 1 well indicated that the well was not in production nor had it been plugged and abandoned per the requirements outlined in Notice of Violation # 1488817388 issued 3/6/2017. I did not identify any work that was performed to bring the well into compliance with the Notice of Violation.
1870151288 2017-08-22 Production Wells No Violations I conducted an inspection on the Mead #1 as a follow up to Notice of Violation # 1488817388. The well last reported production in 2012. The Operator, James Coe, took over ownership in 1995. The well was completed as a pumping well with rods and tubing. The polish rod liner was sitting down on the stuffing box as there wasnt a pumping unit on location. The landowner, Mr. Levi Miller, reported that the pumping unit was removed approximately twenty (20) years ago. The well head was of typical pumping well configuration with the well fluid produced through the tubing and the natural gas was extracted through the production casing. The connection to the flowline was incomplete. A bypass hose connected the 8.625 surface casing and the 4.5 production with the valves to both open. A domestic gas tap and meter was tied into the wellhead production casing. The gas meter readings were at 144100 at the time of this inspection. The access to the wellhead and tank battery was restricted. The tank battery consisted of one (1) 100 barrel stock tank, one (1) in ground concrete brine storage vault and one (1) 2408 horizontal separator with orifice gas meter. The 100 barrel stock had the required I D signage posted and was legible. There was an earthen dike surrounding the tank and vault. There was oil contamination, on the northeast side of the 100 barrel tank, apparently coming from the tank bottom seam. This area was three (3) feet by two (2) feet. I viewed the inside of the 100 barrel and it appeared that the fluid level was at the two (2) inch brine drain level. The brine vault was completely full. The separator was also enclosed within the dike. The gas meter had an old chart, but without any recent readings. There was a one (1) inch line that was a house service line connected to the separator. The well is incapable of production and the Notice of Violation ( # 1488817388 ) remains effective.
2014413740 2017-11-29 Production Wells No Violations Lakeside Oilfield Services was on location to dismantle the tank battery, remove the 210 barrel stock , 100 barrel brine tank and remove and replace the stone pad of the tank battery. The tanks were steamed cleaned prior to removal from the location. The tank battery stone pad was excavated down to yellow clay, an impermeable member of the Platea - Sheffield soil series. The tank battery pad was to be condensed in size since only one (1) 100 barrel stock tank would be utilized. I tested the free water in constructed sump and found its parameters to be as follows: 46 F, pH - 7 and Chlorides ( High Range QuanTab ) ~ 1063 ppm.
