1340879176 2018-12-29 Production Wells Violations Noted I conducted an inspection on this well. The well was equipped with a plunger lift assembly and not in production. There was no annular disposal present. There was an audible tubing packing leak. Production for this well has been submitted as zero from 2006-2017. Tank battery location was comprised of one 100 BBL tank, one 50 BBL tank and a mechanical separator. The diked tank battery contained legible identification. The lease access to the tank battery was not stable. The tubing packing leak needs to be repaired. The well owner shall submit a written plugging schedule to Inspector Shannon Mullins at ODNR Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days. The well owner/operator will be notified if the plugging schedule is approved or denied. The J. Ekenstein #1 shall be plugged within 180 days.
1536250370 2018-08-30 Production Wells Violations Noted I conducted an inspection on this well location. At the time of this inspection, the well was unable to be located due to heavy vegetative overgrowth. The tank battery consisted of one 100 BBL tank, one 50 BBL tank and a mechanical separator. Valves on the separator were closed. The base on the 100 BBL tank was seeping and the heater tube was stained and wet with fluid. The tank battery was diked. The signage at the tank battery was not legible. The lease access was nonexistent. I contacted Fran DeSanctis from Petrox, Inc. Ms. DeSanctis stated that the 100 BBL tank would have the fluid removed and the tank cleaned out. I also requested that Petrox locate the well so that an inspection could be conducted accordingly and to add signage at the well site.
1538408154 2018-10-01 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1536250370. Items that needed addressed for this compliance notice were, emptying 100 BBL tank inside the tank battery and adding identification to the tank battery location. At the time of this inspection, Petrox had emptied the 100 BBL tank. Legible identification has also been added to the tank battery location with current owner information. Compliance notice can be closed.
