I also took photos of the unloading pad and frac tank on the unloading pad . REMEDIAL WORK REQUIRED TO CORRECT VIOLATIONS : REMOVE ALL OIL SATURATED AND BRINE CONTAMINATED SOILS . REPLACE SOIL AND ESTABLISH VEGETATION IN AREAS SOIL WAS REMOVED AND REPLACED . |

Diamond Well Service rigged up the pump truck to establish circulation to set Plug #2. The pump truck operator reached 950 psi and was unable to pump salt water down the tubing. The pump truck operator shut-in down the pump truck and Diamond Well Service disconnected the flow line from the tubing at the well. Next, the rig operator attempted to pull up on the tubing and reached a string weight of 25,000 lbs. After meeting with Tom Wood, contracted geologist for Diamond Well Service, Mr. Wood stated the tubing is probably cemented in the well. Mr. Wood scheduled Appalachian Well Survey for 8:30 am on September 6, 2018 to shoot off the tubing. At this time, the well was shut-in for the rest of the day. |

I spoke with Brandon Williams with Rettew [ environmental consultant ], he explained the work to be performed for the days activity. I took two photos of the unloading pad before any demolition. The contractors began removing soil from the south-west corner of the pump house. After the first truck load had been removed I took two photos of the excavated area. The second truck then arrived and was loaded with soil from the same area. After the second truck had been loaded fluid began flowing into the excavated area from a spot on the north side of the excavated area about 1.5 feet below the surface. I took three photos of the excavated area after the second truck load of soil had been removed. The contractor then placed a plastic pipe vertically in the excavated area to be able to remove the fluid. I took two photos of the excavated area after the area was full of fluid. I performed a test of the fluid flowing into the excavated area with a 1176 quantab strip to find the fluid showed to have 1000 PPM of chlorides in it. A brine truck then arrived and removed the fluid from the excavated area. I took one photo of the brine truck removing the fluid from the excavated area. The brine truck then removed the fluid from the sump inside the impoundment area. After the sump had been emptied I discovered there was a buried tank below the sump pipe. The contractor then began breaking the concrete unloading pad with a jack hammer attachment on the small track hoe. The dump trucks arrived and the contractor removed two loads of the earthen berm on the west side of the unloading pad and some concrete from the unloading pad.
