0746491073 2016-08-26 Final Restoration No Violations On location with DOGRM Inspector Mike Dinsmore. Plugging operations on the well were completed 09/25/2015. The areas affected by well production and the plugging operations have been graded. The affected area has not been seeded. Sparse native vegetation is starting to take hold in the area of the access road. The area around the well is barren. Drainage from the affected area flows north east into an intermittent stream. Erosion and sedimentation was evident. Sediment fines lead up to the intermittent stream. Remedial actions that need to take place: 1. Erosion control measures put into place where the site discharges into the intermittent stream. 2. Area affected by the reclamation of the site needs to be seeded.
0819426809 2013-08-28 Production Wells No Violations Inspection found the wellhead still had a swedge on the 4.5 production casing with a 2 valve which is shut-in . There is no tank battery for this well . The status of the well has not changed since a compliance notice was issued on 2-17-1012 . The well is incapable to put into production .

1190078706 2012-02-17 Production Wells No Violations Complaint#12-001 [ Loretto Accettola ]. My inspection of this well site found no tank battery for well. Further inspection found 4.5 casing with a swedge to two inch with a valve, which is shut. The well is discconnected from the flowline and is dle . ODNR records show that this well had zero production reported since 2005. I contacted Bob Barr Jr. and informed him of the relevant idle well violation and the time frame for the required remedial action needed to get the well in compliance.
1236140928 2015-09-23 Plug / Plug Back No Violations I was on site to witness the plugging of the well pursuant to Chiefs Order # 2013-38. The cement company mixed the bottom plug and displaced the plug through the tubing. The rig crew then pulled the tubing from the well. I then left the site till the next day.
1284809887 2012-08-01 Production Wells No Violations My inspection was relevant to the compliance [ plug or produce ] notice. I found that no activity has been initiated to produce or plug this well. I spoke with Bob Barr Jr. who told me they had applied for a Temporary Inactive (TI) status through our Columbus office. I then contacted Steve Opritza, at our permitting office, who told me he had not received any TI application from Big Sky. He also told me they would be denied due to the well being inactive for so long (since 2005).
1350356921 2012-09-09 Production Wells No Violations My inspection found there has been no remedial work performed at this time .
1415032803 2014-10-16 Production Wells No Violations My inspection of this well found there has been no action to plug this well pursuant to Chiefs Order # 2013-38. I spoke with Bob Barr Sr. , he told me there is no current plan to plug this well due to financial difficultys.
1430545025 2015-09-25 Plug / Plug Back No Violations I was on site to witness the plugging of the well pursuant to Chiefs Order # 2013-38. I arrived on site to the rig crew preparing to run the ripping tool into the production casing. The rig crew ripped the production at 2177. They then proceeded to plug the well to completion. NOTE: The plugging of this well resolves Chiefs Order # 2013-38.
1629611227 2015-09-23 Plug / Plug Back No Violations I was on site to witness the plugging of the well pursuant to Chiefs Order # 2013-38. The rig crew opened the production casing to find there was no pressure on the well. They then opened the valve on the surface casing head also to find there was no pressure on the annulus of the production casing. The rig crew then run into the production casing with the sand line to find an obstruction at 2800. The rig crew then run tubing into the production casing to the area the obstruction was encountered. We left the site till the next day.
1976626678 2017-09-21 Final Restoration No Violations I inspected this former well location for evaluation for final restoration. At the time of this inspection, the former area of the well had been graded and had well-established vegetation, with no signs or remnants of the well or associated production equipment or battery. No further action is needed. Final Restoration is passed.
1985523734 2017-07-14 Final Restoration No Violations At the time of my inspection, native vegetation has become established over most of the disturbed area. The only areas where vegetation has not established itself is approximately 300 feet north of Mechanicsville Road for approximately 150 feet along the east side of the reclaimed access road. The area is heavily shaded by tree canopy. My review of the site and access road indicated no signs of substantial erosion or sedimentation.
2071019107 2016-11-02 Final Restoration No Violations I inspected this former well location to evaluate final restoration. At the time of this inspection, I observed the former well location to be level with some nascent growth of wetland-type vegetation. The access road was level with well established grassy vegetation. I did not observe any signs or remnants of the former well or associated production appurtenances.
