0234739875 2015-09-10 Plug / Plug Back No Violations I arrived on site to find a Diamond Oil service rig rigged up over the well. There was no one on site at this time. After I left the site the service rig crew pulled 10 stems of the rods then left the site.

I then left the site.

0752128746 2015-09-15 Plug / Plug Back No Violations I arrived on site to find the rig crew preparing to run in the well with a overshot in a attempt to retrieve the separated rods and tubing. They started down the well to find the overshot would not go past the surface casing head. They then hooked up to the production casing and pulled the casing out of the surface casing head to find the production casing was collapsed where it set in the surface casing head slips. They could not pull the casing high enough to repair the collapsed casing. They then lowered the casing into the well to point all the hanging weight was off the casing. The rig crew then removed the surface casing head from the surface casing. They placed casing slips on the surface casing and pulled the production casing from the well high enough to repair the collapsed casing. The rig crew then cut the production casing below the collapsed area and welded a bell collar on the production casing. They then lowered the production casing into the well to relieve the hanging weight off of the casing. They then reattached the surface casing head to the surface casing. They then pulled the production casing from the well and placed the slips in the surface casing head and lowered the casing into the surface casing head. The rig crew was preparing to go into the production casing with a overshot on tubing to retrieve the rods and tubing still in the well when I left the site.
0902851524 2017-01-18 Final Restoration No Violations My inspection found the final restoration has not been completed. The flow line has not been removed, the tank pad has not been reclaimed and the stone road to the tank pad area has not been removed.
0917732819 2017-07-14 Final Restoration No Violations At the time of my inspection a riser pipe remained protruding from the ground next to the tank battery pad. Fill material has been placed on the tank battery pad area. There was no surface disturbance noted that would indicate that the flow line has been removed. Access road remains, a landowner waiver for the road has not been submitted at this time.
1073641816 2015-09-11 Plug / Plug Back No Violations I arrived on site at 9 a.m. there was no one there. The rig crew arrived at 9:30 a.m.. I then had the rig crew uncover the surface casing head. The valves were closed to the annulus of the production casing. They opened the valves to find no pressure on the annulus of the production casing. The rig operator then attempted to pull the tubing from the well when the tubing separated. They pulled the 2 tubing to find there was a change over sub to 1.5 tubing. The well began gassing. They the run back in the well with the tubing and packed off the annulus of the tubing. They then run lines to the above ground steel tank and opened the valve from the tubing annulus and let the well vent into the steel tank. We then closed the valves from the tubing annulus and left the site till Monday morning.
1083781079 2011-12-15 Production Wells Violations Noted Complaint#11-006, Loreto Accettola. I arrived on site to find service rig, a semi with a low boy trailer and a small track hoe parked in lease road entrance by tank battery. Upon further inspection, I found a 36 culvert had been removed at the stream crossing and placed on the well side of the lease road. A 20 section the electic wire to the well was only 5 or less above ground level. Approximately 80% of the flowline from the well to the tank battery was on the ground surface. There was exposed wiring in the elictric box at the pumping unit. There was no identification posted at tank battery area. ODNR records show well did produce in sufficient amounts in 2010. REMEDIAL WORK REQUIRED: Establish proper identification at tank battery area, bury flow line to at least 24 below grade from tank battery to well. Raise electic line to a safe height, cover and secure exposed wiring at electic box at pump-jack . NOTE: After conversation with landowner I learned he had the sheriff remove the well owners workers from the property when they were attempting to service this well. Also, he is in litigation with the well owner. I notified them that I need to be contacted if remedial work cannot be completed by deadline date due to pending litigation.
1345733856 2012-08-01 Production Wells No Violations My inspection of remedial work relavent to the compliance notice: I found the identification has been posted, the electric wire has been raised to 8 above ground, the exposed wires at electic box, by the pumping unit, has been secured from access. The exposed flow line has not been properly buried at this time. I spoke with Bob Barr Jr. who told me they are in litigation with landowner and are unable to access the property to bury flow line at this time.
2049175775 2015-09-16 Plug / Plug Back No Violations I arrived on site to find the rig crew was able to recover the tubing and was preparing to pull the tubing from the well. They began pulling the tubing from the well to find there was rods hung in the tubing. I was on site to witness the rig crew remove all of the rods and tubing from the well.
2120689234 2015-04-02 Production Wells No Violations Complaint# 3499 follow-up. My inspection found this well is not in production at this time. There is a large concrete block blocking access to the well. Bob Barr Sr. told me the landowner is denying him access to the well. He also told me they need to get a service rig on the well to put the well back into production.
2126537847 2015-09-14 Plug / Plug Back No Violations I arrived on site at 8:30 am. I learned the company had pumped 80 bbl.s of fluid into the annulus of the tubing. The well was not emitting any gas when I arrived on site. The rig crew then pulled 360 of tubing where it had separated. Left site till the next day.
