0034671582 2013-08-28 Production Wells No Violations Inspection of the tank battery area found the company has cut down all the weeds inside the earthen dike walls . The well is still not plumbed into the flowline at the wellhead . The well is still not capable to being put into production .
0047151994 2016-10-27 Production Wells No Violations I arrived on site to find Bob Barr Sr. and 4 crewmen were on site. The last effort to recover more 1.5 tubing resulted in recovering 3 more joints. The pipe tally of the recovered tubing from the well was 2988.12 feet. The depth was sufficient to adequately plug the well. They then removed the 4.5 production casing from the well. After removing the production casing they started running the 2 tubing to the total depth reached from recovering the lost 1.5 tubing. They were running the 2 tubing in the well when I left the site.

I took two photos of the 4.5 production casing setting in the casing slips. The rig crew was in process in repairing the power tongs. They then started to pull the production casing. They had 9 joints of 4.5 casing removed from the well when I left the site.

0249104665 2016-09-08 Production Wells No Violations I arrived on site to find Tim Martin [ Excavating contractor ] grading the wellsite to allow any rainfall to run off the site. There was no rig crew on site. I visited the site a couple hours later to that Tim Martin was still grading the site. There was no rig crew on site. I spoke with Tim, he told me he was not sure if the rig crew was going to show up today. I waited for a while when I called Bob Barr Sr., he told me he was not sure when or if the rig crew was going to show up today. I a while longer when the rig crew did not show up I left the site. I told Tim if the rig crew did show up to have them contact me.

0471314317 2016-11-01 Production Wells No Violations I arrived on site to find Bob Barr Sr. and 4 crewmen loading the recovered 1.5 tubing onto the semi trailer to transport to a salvage facility. They had run the 2 tubing to the total depth reached from recovering the lost 1.5 tubing which was 2988 feet. I took one photo of the working pit with the liner installed. Petroset cementing company [ cement contractor ] arrived and set up to plug the well. They pumped 20 barrels of water to obtain circulation of the well. I took two photos of the well circulating into the working pit. They then began mixing a slurry of cement and started displacing the cement through the 2 tubing to the total depth of the tubing. A 300 plug was displaced for the bottom plug. The rig crew then pulled the tubing to a depth of 868 feet. The cementers then displaced a 468 foot plug through the tubing. The rig crew then pulled the tubing to a depth of 400 feet. Then the cementers displaced cement till it circulated to the surface. They had good circulation through the entire plugging operation of the well. The rig crew then removed the remaining tubing from the well. The fluid in the well bore remained static. The rig crew was in the process of rigging down the service rig when I left the site.
0481959787 2016-10-11 Production Wells No Violations I arrived on site to find Charles Mosio and two crewmen running the 2 inch tubing with the overshot into the production casing. NOTE: They had run the 4.5 production casing back into the well with a homemade centralizer on the bottom joint on 11-10-2016 which was a holiday, I did not witness the placement of the production casing. They set down on the 1.5 tubing to attempt to attach the overshot. I took one photo of the rig crew pulling the tubing from the well. When the overshot was pulled through the tubing slips there was 1.5 tubing in the overshot. I took one photo of the 1.5 tubing with the overshot attached setting in the tubing slips. When the overshot was removed from the 1.5 tubing I took one photo of the collar on the 1.5 tubing. They the began pulling the 1.5 tubing to find they had recovered 20 more joints. The threads on the bottom joint was missing. They began to run the tubing with the overshot back into the well when I left the site.
0492798915 2018-05-15 Final Restoration No Violations I inspected this well location as a follow-up to plugging and abandonment that had been completed previously on 11/01/2016, but final restoration had remained not complete. At the time of this inspection, I found that the former well location had been re-graded with no additional activity needed. The former tank battery remained in place, and must be removed and the area restored before final restoration can be granted.
0576905316 2017-09-27 Production Wells No Violations I inspected this well location as a follow-up to an earlier inspection in which the recently plugged well was noted to be leaking a small amount of gas from the top surface plug. The plugging contractor (Charlie Moisio) was subsequently directed to apply some additional ready-mix cement to the top hole. At the time of this inspection, the top-most cement plug was at the level of the surface casing with no signs of gas leakage.

0771329001 2017-09-20 Plug / Plug Back No Violations I inspected this well location, along with NE Region Supervisor Tom Hill, as part of evaluation of the recently completed plugging and abandonment activity. At the time of this inspection, we observed the following: an 8.625-inch surface casing, which had been cut down to approximately 30 inches below the existing grade, subsequent to plugging. The annulus was full of water, and the top surface plug was measured to be approximately 14 feet below the top of the casing. There was a small, but visible leakage of gas bubbles within the casing annulus. We discussed various options for mitigation of the leaking gas, and recommended that an attempt be made to seal the leak with top-loaded cement. We agreed to return once this had been executed.
0832937134 2016-10-03 Plug / Plug Back No Violations I arrived on site to find Charles Mosio and two crewmen running the two inch tubing with the overshot into the production casing. They set down onto the 1.5 tubing. They set down onto the tubing several times. They then began pulling the tubing from the well. They had 14 joints out of well when the tubing became hung in the well. They pulled on the tubing for several minutes when the tubing jumped out of the well two feet. They began to pull on the tubing to find the tubing was free. They then pulled the tubing with the overshot from the well to find they had not recovered the 1.5 tubing. A total of 44 joints of two inch tubing with the overshot was used in an attempt to recover the lost tubing in the production casing. I took one photo of the overshot hanging over the well. Charles was preparing to run the casing scraper into the well when I left the site.
0833789338 2015-10-02 Plug / Plug Back No Violations Follow-up pertaining to Chiefs Order # 2013-37. I arrived on site to find the plugging contractor had removed the service rig and equipment from the site.
1038663108 2014-01-09 Production Wells No Violations I met with Northeast Region Supervisor Tom Hill and Inspector Jerry Kohl at the above location and performed an inspection of the location. The 2 valve coming frm the well head was closed the gas chart had not been changed for sometime. The valves at the wellhead were closed.
1085607785 2015-09-29 Plug / Plug Back No Violations I was on site to witness the plugging of the well pursuant to Chiefs Order # 2013-37. I arrived on site to find the rig preparing to pull the tubing from the production casing. The rig crew pulled to joints of tubing to find the production casing was open. They then run into the production casing to find an obstruction at 250. They then run into the well with a impression tool on tubing to the obstruction. When the impression tool was pulled from the well it showed the tool had set down on a collar on the remaining tubing in the production casing. I left the site till the next day.

1199940555 2016-09-14 Plug / Plug Back No Violations I arrived on site at 10 am to find no one was on the site. At 11:50 pm Bob Barr Sr. and two crewmen arrived on site. After servicing the rig and getting everything into operating condition they began running the two inch tubing with a overshot into the production casing at 12:50 pm. At 2:00 pm when the seventeenth joint was being lowered into the well fluid started displacing from the production casing and then flowing into the surface casing through an open valve. When the 27th joint of tubing was 3/4 of the way in the well the tubing hit an obstruction stopping the tubing from going any further down the well. Bob tried several times to lower the overshot over the obstruction with no success. The rig crew began pulling the tubing from the well when I left at 3:05 pm.

They set down on the 1.5 tubing several times then started pulling the tubing with the overshot from the well. When I left the site they were still in the process of removing the tubing and overshot from the well.

The working pit was full of fluid to the point of overflowing. The surface casing has not been cut below grade at this time. I spoke with Charles Mosio and he told me he would empty the working pit and close it tomorrow.
1321716210 2012-08-01 Production Wells No Violations My inspection was relevant to the compliance [ plug or produce ] notice. I found that no activity has been initiated to produce or plug this well. I spoke with Bob Barr Jr. who told me they had applied for a Temporary Inactive (TI) status through our Columbus office. I then contacted Steve Opritza, at our permitting office, who told me he had not received any TI application from Big Sky. He also told me they would be denied due to the well being inactive for so long (since 2006).
1342447713 2012-02-17 Production Wells Violations Noted Complaint# 12-001, [ Loretto Accettola ]. My inspection of this well found the tank battery equipped with one 100 barrel steel production tank, one seperator and no gas meter. Further insection found the well equipped with a plunger lift and 1.5 tubing in 4.5 casing with valve on tubing which was shut. Also, the flowline is discconnected from the well. ODNR records show that this well had zero production reported since 2006. This well is idle. I contacted Bob Barr Jr. and informed him of the relevant idle well violation and the time frame for the required remedial action needed to get the well in compliance.
1355274116 2016-10-05 Production Wells No Violations I arrived on site to find Charles Mosio and 2 crewmen pulling the 2 inch tubing with the overshot from the well. When the overshot was removed from the well they found they had not recovered anymore 1.5 tubing from the production casing. They changed the overshot to the casing scraper and then began running into the production casing. They went through the tight area in the production casing several times and then began removing the tubing with the casing scraper. When the tubing was removed from the well they changed the casing scraper to the overshot and began running back into the production casing. I left the site at this time.

Upon my inspection of the wellhead , I found the valve that is located between the rabbit catcher and the tubing is closed and is not allowing gas or oil to flow to the separator . The well is not in production at this time .

There has been a ladder established on the production tank to access the top of the tank . Further inspection of the wellhead found company has put a rabbit catcher on the tubing and plumbed it into the flowline . The valve from the tubing to the flowline is closed at this time . The well was not in production at the time of my inspection .
1452945045 2016-09-16 Plug / Plug Back No Violations I arrived on site to find Bob Barr Sr. and three crewmen running two inch tubing into the production casing with a spear in an attempt to retrieve the lost tubing in the well. I took three photos of Bob Barr Sr. and the rig crew actively running the tubing into the well. At 12:15 pm they set down on the obstruction. They set down on the obstruction a few times when they began pulling the tubing from the well. At 1:15 pm they had removed the tubing with the spear attachment to find they had not successfully secured the lost tubing with the spear. I took one photo of the spear attachment.
1473349384 2016-09-06 Plug / Plug Back No Violations I was on location to oversee plugging operations that are required by Big Sky Energy Incorporated and mandated by the Ashtabula County Court of Common Pleas. No plugging operations will be permitted to commence until Big Sky submits proof that they have obtained the proper liability insurance as required by Chapter 1509.07 of the Ohio Revised Code. My inspection found that Big Sky has cleared and graded the location to make it accessible for plugging operations. They have spotted a pipe trailer with 2.375 tubing on the east side of the wellhead and a single pole service rig to the west. Two (2) rig hands were on location attempting to service the rig and make it capable of performing the job. Bob Barr (Big Sky) arrived on location and told me he has obtained and submitted the required insurance policy to the State. No operations may commence until the policy is approved by the Surety Section in Columbus. Ashtabula County Inspector Jerry Kohl arrived on location and will assume the oversight of plugging operations for this well.
1474049144 2016-09-06 Production Wells No Violations I arrived on site to find a service rig was rigged up over the well bore. Mike Dismore [ Oil & Gas Inspector ], also Bob Barr Sr. and a rig crew was also on site. The rig crew was working on the service rig. I spoke with Bob Barr Sr., he told me they were not going to do any work on the well today.
1474563311 2016-09-21 Plug / Plug Back No Violations I arrived on site to find Bob Barr Sr. and two crewmen were preparing to run impression block into well with the swabbing line. I took two photos of Bob and crewmen running impression block into well. They set down on the obstruction with the impression block and began to remove the swab line. I took two photos of Bob and crewmen removing the impression block. Upon removal of the impression block, the impression indicated pipe threads. They began to run the overshot into the well. I took two photos of Bob and crewmen running the tubing into the well. Upon setting down on the obstruction they tried several times to latch onto what they believe was 1.5 tubing. Their attempts failed. They had eight joints of tubing removed from the well when I left the site.
1477923951 2016-09-28 Plug / Plug Back No Violations I arrived on site to find Bob Barr Sr. and four crewmen pulling tubing from the well. When they reached the overshot there was 1.5 tubing in the overshot. I took one photo of the tubing in the overshot. I took one photo removing jaws from the overshot. I took one photo of 1.5 tubing in the tubing slips. They the pulled the 1.5 tubing to find they had retrieved three joints of tubing. I took one photo of the smashed threads on the last joint. The rig crew then rigged up the swab line with a impression block. They run the impression block into the 4.5 casing to they could not get past 840 feet. They then pulled the impression block. They impression block did have any markings on it to indicate what the obstruction might be. They the run the overshot into the 4.5 casing to find they could not get past 840. They then shut down for the day.
1478100364 2016-11-02 Plug / Plug Back No Violations I was on location to inspect this well undergoing plugging and abandonment. At the time of this inspection, placement of plugs had been completed on 11/01/2016 and activities to complete plugging and abandonment were continuing. Upon inspection, I observed water at a depth of 3 feet in the surface casing annulus. There was no sign of gas production or fluid loss. Crews ran a weighted line and found the top of the surface plug to be 18 feet below the casing head. Continued work will include cutting of the surface casing to 30 inches below grade and placement of a secure, metal ID covering plate atop the open surface casing, followed by restoration work.
1479832175 2016-10-04 Production Wells No Violations I arrived on site to find Bob Barr Sr., four crewmen and Charles Mosio pulling the tubing with the casing scraper from the well. I took one photo of Bob Barr Sr. running the service rig. When the tubing had been removed from the well they changed the casing scraper to the overshot and then began running back into the production casing. They set down on the 1.5 tubing when it dropped down the well. They run the 45th joint of two inch tubing into the well when they set down on the tubing with 15 feet left on the joint of tubing above the casing slips. When they started to pull the overshot from the well there was more weight on the string of tubing for two feet when they lost weight of the tubing. They then set down with the overshot a few more times when they began pulling the tubing with the overshot from the well. When the overshot was pulled through the tubing slips there was 1.5 tubing in the overshot. I took one photo of the 1.5 tubing in the overshot when it was hanging above the tubing slips. After they removed the overshot from the 1.5 tubing they pulled the tubing to find they had recovered one joint of 1.5 tubing. I took one photo of the bent 1.5 that had been recovered when it was lying on the ground. They were beginning to run the casing scraper into the well when I left the site.

They set down on the tubing several times and then began pulling the tubing with the overshot from the well. They had 3/4 of the tubing removed from the well when I left the site.
1486486818 2018-02-07 Final Restoration No Violations I inspected this well location as an evaluation for final restoration. Plugging and abandonment was completed on 11/1/16. Since that time., the site had been re-graded to match the surrounding grade. The landowner had indicated that no re-seeding was needed. The tank battery was still in place and removal was pending. I contacted the contractor, Charles Mosio, regarding the status. He indicated that he was working to complete the restoration but did not report a definitive schedule or estimate for completion. The defective/leaking casing condition that prompted the compliance notice has been resolved with the plugging and abandonment. The requirements for final restoration remain not complete pending the removal and restoration of the tank battery.

1565159036 2016-09-29 Plug / Plug Back No Violations I arrived on site to find Charles Mosio and two crewmen rigging up a casing scraper onto the 2 tubing to run into the 4.5 casing. I took two photos of the casing scraper. They then run the casing scraper into the well to 840 where they hit an obstruction preventing them from going any deeper. Charles the raised the tubing and dropped it several times when he finally went past the obstructed area. He then did this several times till they were able to go past the obstructed area easily. They then pulled above the obstructed area and shut down for the day.

1729361036 2015-09-30 Plug / Plug Back No Violations I was on site to witness the plugging of the well pursuant to Chiefs Order # 2013-37. I arrived on site to find the rig crew was in the process in repairing a disabled bulldozer. After the bulldozer was repaired the rig crew then run a overshot tool on tubing into the production casing to attempt to retrieve the remaining tubing in the well. They were able to retrieve 6 joints of tubing from the well. They then shut-down till the work day.
1739248776 2017-09-21 Plug / Plug Back No Violations I inspected this well location as a follow-up to my inspection yesterday (09/20/17). At the time of the previous inspection, I along with ODNR NE Region Supervisor Tom Hill had met with plugging crews at the site of the recently plugged and abandoned well. We observed that there was a small but visible leakage of gas from inside the open surface casing annulus, which was otherwise filled with water. We had recommended that the crew mix and top load additional cement to seal the leak. At the time of this inspection, I observed a small amount of effluent gas in the open casing. I contacted the plugging crew and advised that they make an additional attempt to seal the leak with cement and we agreed that I would re-visit the site once that effort had been implemented.
1823819811 2017-07-14 Final Restoration No Violations At the time of my inspection the surface casing which is full of water up to the side ports, remains above grade. Probing inside the casing indicates approximate cement top at approximately 24 inches below grade. Slight bubbling inside the casing can be seen along the west and east side of the casing. A Kubota excavator is on location. Recent earth work has been done along the north side of the access road and across the site east of the well to allow for drainage. Site has not been reclaimed at this time. Well remains above grade and not tagged.

1933034653 2015-10-01 Plug / Plug Back No Violations I was on site to witness the plugging of the well pursuant to Chiefs Order # 2013-37. I arrived on site to find the rig crew preparing to run into the well with the overshot. They were able to remove more tubing from the well. The bottom joint revealed signs the tubing had separated again. Calculations of the amount of the tubing recovered from the well found the top of the remaining tubing in the production casing should be around 1250. The rig crew then run the overshot back into the well to find they could not get past 834. They attempted to get past 834 several times with no success. They then shut down for the day.

They then set down on the obstruction with the overshot a few times. They then pulled the tubing with the overshot from the well to find they had not recovered anything from the well. They changed the slips in the overshot with larger slips. They were running the tubing with the overshot into the well when I left the site.
