There also has been no action to establish a earthen dike around the tank battery. Since the company replaced the production tank there has been no identification posted for this well.
0061081150 2015-12-02 Production Wells No Violations I conducted a compliance notice follow-up inspection of this well. This is also in conjuction with the complaint# 4262 involving Joe Simak. The valves at the wellhead were closed. The electric line was still disconnected from the main line at the telephone pole as well as at the well location. The dike around the tank battery needs reconstructed for sufficient storage capacity. The storage tank that was replaced was still on the north side of the tank battery area. Identification still needs posted at the tank battery.

There was still no dike established around the production tank. There was no identification posted for this well at the tank battery location.

My inspection found the electric line was severed from the main line at the telephone pole on the south side of the tank battery area. The electric line was on the ground from where it was severed to the well location. There was no earthen dike wall of sufficient height established around the storage tank. The storage tank that was replaced was still on the north side of the tank battery area. There has been no identification posted at the tank battery area. There was no power supply to the pumping unit at the well. This well was not in production at the time of my inspection.

0964156771 2017-10-24 Production Wells No Violations I conducted a compliance notice follow up inspection for compliance notice 1366634829 and complaint #4262. At the time of this inspection, there has been no changes to the well or tank battery locations. The well was equipped with a pumpjack and the electric line remains disconnected from the pumping unit. The well location was still very overgrown with weeds. The tank battery still consisted of a 100 BBL production tank and a mechanical separator. The base of the 100 BBL tank was stained with old oil and has not been cleaned up. There was another 100 BBL tank located north of the tank battery. The dike needs to be rebuilt and signage needs added to the tank battery location. Vegetative maintenance was also needed at the tank battery. The lease access was stable.

1356207874 2018-03-06 Production Wells No Violations I inspected this well pursuant to Chiefs Order #2016-300 issued for lack of insurance. The well and associated battery are located on a rural farm parcel. At the time of this inspection, I noted the following: a wellhead with pump jack system in a farm field. The pump was idle. The area around the pumping unit was overgrown. The fittings were secure with no signs of leakage. The associated tank battery was comprised of one 100-barrel production tank, and horizontal separator. There was a discarded 100-barrel tank adjacent to the battery, not part of the active system. The secondary containment was mostly overgrown with vegetation, but was free from signs of spills, or leaks. There were no signs that the well was being produced. There was a charter recorder that was not active.
1366634829 2013-03-26 Production Wells Violations Noted Complaint [ Joe Simak ] , My inspection of well found tank battery contained one 100 barrel steel tank and one seperator . The tank battery is located next to small stream on west side five feet away . There was no dike wall on creek side of tank battery . I tested water in standing water between stream and tank battery with 1176 quantab strip which showed over 900 ppm chlorides . Further inspection of electric power supply found the electic wire has been cut after electric meter , the wire is lying on ground from tank battery to well site along stream in wooded area , the wire does not go to well from stream over field to pump-jack unit for 100+ feet . Inspection of pump-jack unit found small trees and brush growing around and through pump-jack unit . Inspection of wellhead found valves to flowline were open , also there is dried paraffin around wellhead one foot in diameter . ODNR records found company reported 16 barrel of oil and 41 mcf of gas production for 2010 . REMEDIAL WORK REQUIRED : Remove 100 barrel steel production tank to determine if integrity of tank has been compromised , establish earthen dike around production tank of sufficient size to contain contents stored in tank , remove wire lying on ground from tank battery to well site area , submit production reports for well for 2011 and 2012 , produce or plug well .
1376393594 2013-08-12 Production Wells No Violations I was on location as a follow up to complaint #4762. The power line leading back to the pump jack had been removed. The pump jack is over grown with weeds. The tank battery consists of a 100 BBL production tank and a mechanical separator. There is another 100 BBL sitting in front of the tank battery. The only identification sign is on the pump jack and needs to be updated. I spoke with Jamie from Big Sky and he stated that someone stole the downed power line. He also stated that the well is scheduled to have a service rig move on the location and to have the needed repairs done this fall. I advised him to have the old 100 BBL removed and to get the proper identification on the location.

Inspection of well found company has not done any remedial work to comply with the compliance notice issued on 3-26-2013, due date was 5-24-2013. I have contacted the company several times but have had no response to comply with the compliance notice.
1544795435 2018-12-13 Production Wells No Violations I conducted a compliance notice follow up inspection for compliance notice 1366634829 and complaint #4262. At the time of this inspection, the well was not in production. The well was equipped with a pumpjack and the electric line remains disconnected from the pumping unit. No motor was present on the pumping unit and the well location was still very overgrown with weeds. The tank battery still consisted of a 100 BBL production tank and a mechanical separator. The base of the 100 BBL tank was stained with old oil and has not been cleaned up. There was another 100 BBL tank located north of the tank battery, disconnected from the other production equipment. The dike still needs to be rebuilt around the tank battery. Legible identification still needs added to the well location.

There remained no unit identification, and oil staining around the base of the 100-barrel stock tank that had been observed previously remained. The system elements were in disrepair.

The earthen dike is not of sufficient size to contain capacity of tank stored within earthen dike , also electic line to well is still lying on ground . I called Jaimie and told him of remedial work needed to comply with violations .
