0033146088 2015-07-10 Production Wells Violations Noted My inspection of this well found the tank battery area is overgrown with 8-9 foot tall fragmite grass. There is no identification posted at the tank battery area. While walking to the well location I come across a active gas leak 200 feet south of the tank battery. At the well I found fresh oil and paraffin on the ground around the well casing in a area two feet by eight feet. I spoke to Brian the well tender and told him of the gas leak. REMEDIAL WORK REQUIRED TO RESOLVE THE VIOLATIONS: REPAIR THE GAS LEAK, REMOVE ALL OIL AND PARAFFIN ON THE GROUND AROUND THE WELL, ESTABLISH IDENTIFICATION AT THE TANK BATTERY AREA.
0173257310 2018-05-15 Production Wells No Violations I inspected this well pursuant to open compliance deficiencies. At the time of this inspection, I noted the following: a wellhead with pump jack system in a clearing in an otherwise wooded area. The pump was idle. There was residual oil from a previous leak at the wellhead that remained. There was gas leaking from the stuffing box fitting. The overhead electric line for the pump jack system was laying on the ground, having been struck by falling branches. The power supply had been disconnected. The associated tank battery was comprised of one 210-barrel production tank, and horizontal separator. The tank was empty. The secondary containment was mostly overgrown with vegetation, but was free from signs of spills, or leaks. There were no signs that the well was being produced. There was a chart recorder that was not active. The access road to the well was heavily overgrown and passable by foot only. The battery appears to be shared with the Korosy #2. There was an active domestic tie-in at the Korosy #2 (#2--0616) wellhead.

1436581461 2015-08-18 Production Wells No Violations I was on location for a follow-up inspection in reference to compliance notice no. 33146088. During my inspection, I found the tank battery overgrown with 8-9 phragmites. There is a pathway to the mechanical separator. There is no identification posted. I observed fresh oil and paraffin on the wellhead and on the ground in a 2 x 8 area. Photos were taken and are available upon request.
1813297033 2014-04-28 Production Wells No Violations I inspected this well to determine if the company is in compliance with Chiefs order # 2014-43 . Upon my inspection I found the chart on the gas meter displayed the date of 4-23-2014 . The chart also indicated this well is in production . While walking to the well location I come across a gas leak in the flow line about 200 south of the tank battery on the west side of the lease road . There is also fresh oil about 6 in diameter around the gas leak location . Further inspection of the well found fresh oil lying on the ground around the wellhead and fresh oil lying on water about 70 south of well location . I immediately contacted Bob Barr Jr. by telephone and told him of the gas leak and the oil spill at the well location . Bob told me he would have someone come out as soon as possible to address the compliance issues . Also there is not a legible identification sign posted at the tank battery location . This well is not in compliance with Chiefs order # 2014-43 . .

I inspected this well location as part of legal proceedings. As such, the following deficiencies had been noted: leakage of oil at the wellhead, and lack of identifying signage. At the time of this inspection, I noted the following: a wellhead with pump jack system and associated battery on a larger, rural parcel. The battery was comprised of one 210-barrel, steel production tank and horizontal separator. Fittings and connections at the both the well head and battery were secure but prior leakage of oil around the base of the wellhead remained. ID signage was present but not complete (lacking complete API permit #). The access road to the well was heavily overgrown with vegetation, as well as the areas surrounding the battery.
