The company then backed off the rods to recover 18 stems. The company then backed off the tubing to recover 1506 feet of tubing. We then left the site till in the morning.

1424706030 2015-02-11 Production Wells No Violations My inspection of this well is in response to a request from Megan Delisi in the Columbus office to perform a follow-up to the Chiefs Order # 2013-156 issued on 9-17-2013 ordering Belvoir Energy Inc. to plug the well. My inspection found the well has not been plugged. There is one 200 bbl steel tank at the tank battery location. The well has only 4.5 production casing with a swage to two inch with a valve which is closed. The well is disconnected from the flow line. I spoke with Chris Pirigy with Belvoir Energy Inc. , He told me they will make arrangements to plug the well.
1578800841 2015-07-22 Plug / Plug Back No Violations I arrived on site to find the company starting to run a overshot tool on 2.875 inch upset tubing into the production casing. The company set down on the rods at 598 feet. They then attached to the rods with the overshot tool. They then backed the rods off to retrieve 42.25 rods. The company then run another overshot tool in the production casing to attempt to retrieve more tubing from the well. They set down at 1500 feet. After several attempts they were unable to retrieve any more tubing from the well. There was no gas emitting from the well. The company then pulled the tubing and overshot tool to find signs on the tool they were setting down on something but were unable to attach the overshot tool to the remaining tubing in the well. The company then run into the production casing with the casing rip tool. They were able to rip the casing at 1463 feet. They pulled the production casing from the well bore. I left the site.
2055982621 2015-07-20 Plug / Plug Back No Violations I was on site to witness the company try several times to free the tubing and rods which was hung in the production casing. They tried to pump fluid down the tubing when the pressure raised to 1000 psi and held with no loss of pressure. They then pumped fluid down the annulus of the tubing in the production casing when the fluid started circulating from the surface casing which indicated the production casing had lost integrity. While circulating fluid there was a change in the color of the fluid after pumping 14 barrels which indicated there was a hole in the production casing or the casing was separated at about 250 feet. The company then run rods down the tubing to find the separated rods at 70 feet. The company was able to attach to the rods, they then tried to pull the rods to find they were hung in the tubing. The company then pulled tension on the rods and left the site till in the morning.
