0123735174 2018-03-13 Production Wells No Violations I inspected this well pursuant to Chiefs Order #2016-300 issued for lack of insurance. The well and associated battery are located on a wooded, undeveloped parcel. At the time of this inspection, I noted the following: a wellhead plunger lift in a small wooded clearing. The main flow-line valve was closed. The well was not in production. The fittings were secure. I noted some leakage of gas around the surface casing, as evidenced by bubbling gas in standing water. The associated tank battery was comprised of one 2100-barrel production tank, and horizontal separator. There was a second, separator unit, not in use. It was not apparent what, if any, well the second separator was associated with., The tank was empty. There was some visible staining around the heater tube inlet from prior leakage. There was a chart record box that was not active. I walked the areas around the well and battery. The areas were covered with several inches of snow obscuring the ground surface. I did not observe any signs of problems.
1432849704 2007-04-26 Production Wells No Violations Case#07-002 , Mr. Lavern Goodge . Inspection collapsed 100 bbl tank laying just outside dike area . Reason for tank explosion is unknown at this time . Clean-up has already been done . NO VIOLATIONS , RESOLVED .
1814723113 2019-01-24 Production Wells No Violations I conducted an order follo-up inspection for Chiefs Order #2016-300 issued for lack of insurance. At the time of this inspection, the well was equipped with a plunger lift assembly and not in production. The tank battery was comprised of one 210 BBL production tank, and a mechanical separator. There was a second, separator unit, not in use. There was no chart in the chart box. I did not see any leaks or issues but at the time of this inspection, the ground was covered with several inches of snow and ice. No legible identification was present at the well or tank battery. Lease access was only stable to the tank battery.
