0902416885 2018-07-09 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1530033901. At the time of this inspection, the tank location has been cleaned up of any free product and the tank removed from the site. The adjacent area of impact along Hall Road has been cleaned and scraped but still needs to be reclaimed. The operator shall: Reclaim all areas affected by the release.
1530033901 2018-06-25 Production Wells Violations Noted On location due to a release of oil and brine. I met with Emergency Operations Coordinator William Romeo on location. The release was due to structural failure of the production tank that is on location, specifically the tank bottom. Oil and brine impacted an area approximately 500 feet in length by 30 feet in width. The area of heaviest impact was at the tanks location. The release traveled west from the tank adjacent to Hall Road. Brett Dietrich was on location actively working on remediating the spill. Philip Dietrich was also on site. Philip indicated that he knew of the release on 06/23/2018. He had mobilized resources to start clean-up at that time. The division was not notified until 06/25/2018 at 10:56 am. Both Brett and Philip were informed that a Notice of Violation will be issued for violations of 1501:9-8-02 (A) (4) & (8) failure to notify the division of an incident and 1509.22 (A) placing brine and crude oil on the ground. Violation of 1501:9-8-02 is non-remedial, Reportable Incident Reference Form will be sent with this notice. The operator shall: continue remediation of the spill until complete. Remove the production tank and reclaim all areas affected by the release.
1539615980 2018-10-15 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1530033901. At the time of this inspection, the tank location has been cleaned up of any free product and the tank removed from the site. The adjacent area of impact along Hall Road has been cleaned and scraped. Area adjacent to Hall Road at the location shows vegetative growth and reclamation. Compliance notice closed.
