0272198090 2015-03-30 Production Wells No Violations Inspection to determine if the operator has taken any action to bring the well into compliance with the compliance notice issued on 9/22/14. The well was found in the same condition as it has been. The valve on the backside of the production casing was open, with 15 psi indicated on the gauge on the flow line. The well is unidentified. No production is being reported and the operator hasnt updated ownership of the well. The owner is not in compliance and a Chiefs Order will be prepared.
0576832717 2015-12-02 Administrative Inspection No Violations The registered owner in Michigan provided documentation to the Division that the company was sold to Philip and Bill Caldwell in New Philadelphia, OH. Phil is now deceased. Bill and Justin Caldwell are listed on incorporation documents on file with the Ohio Secretary of State as of 2005. I attempted to contact Justin at PAC Drilling but was unsuccessful. I spoke with Gus Zoppi, who was originally involved with the Michigan and Ohio corporations but is no longer. He advised that Bill Caldwell is now the sole party responsible for Tuco and that Bill sold the company, its wells and its lease rights to a shale company. No transfers have been recorded with the Division. Courthouse records in Ashland also do not show a transfer or sale record for the well beyond Tuco.
0589143722 2008-11-19 Production Wells No Violations Requested by Patty Nicklaus to investigate well permit number and owner. Well is bonded under TUCO Partnership but was sold to landowner, Dana Reynolds. Well was not transferred to Mr. Reynolds. It is set up for domestic house gas. Well is idle. All valves are closed at well head. All production equipment has been removed and a swedge is screwed in 4.5 casing. Cold,20 degrees.
1235544975 2014-09-19 Production Wells Violations Noted There is a swage on the production casing. The valve on the surface-production annulus is open. The flow line valve is closed at the inlet to the drip. 15 psi is indicated on the pressure gauge on the flow line. The operator states that the well was transferred to Bill Caldwell. Mr Caldwell was contacted via phone and denied ownership of the well and then stated that if he had owned it, all of his interest was sold to B3 Minerals of Canton. B3 appears to be out of business and is not a registered company. The well is not in production and is not being tended. The well needs to be plugged and the location restored by 3/22/15.
1313114161 2016-02-18 Production Wells No Violations The valve on the production casing is open. About 90 psi is indicated on the first gauge. After the regulator there is 15 psi on the flow line. The surface production annulus has been shut in. There are no tracks in the snow around the well. Photos of the well were taken. The nearest residence received a propane delivery during inspection. I was unable to make contact with any of the property owners around the well. Business cards were left. Nobody is claiming ownership of the well, the registered owner company split up and the landowner is unable to be located. The property is currently held by an out of state trust; Dana Reynolds may be deceased. A chiefs order draft was submitted to the legal section for enforcement. The well is unidentified.
1437385181 2014-07-21 Production Wells No Violations The well has a swage on the production casing with a gate valve on it. There is a plastic flow line connected to the well and a pressure gauge which is broken. The well appears to be shut in. There is an odor of gas around the well but there is no audible or visible sign of leakage. There are no tanks connected to the well and it is shut in. Per an inspection by Harold Moravy, the well was sold to Dana Reynolds, the landowner. There is no ID at the well and it is not in production. Tuco Partnership was dissolved. Per Jim Shuman of Production Alliance, Inc, Bill Caldwell of Dennison, OH was the responsible party for Tuco Partnership. The well needs to be plugged, however the current ownership of the well needs to be determined prior to issuance of a plug notice.
1995849054 2016-06-20 Production Wells No Violations A request to inspect the well was received from Cherice Jackson of the Divisions Columbus office. Current photographs were taken. The condition of the well remains unidentified. The well is equipped with 4.5 inch production casing, 8.625 inch surface casing and a conductor casing of undetermined diameter. The production and surface casings are tied in to a regulator showing 15 psi on a gauge. The surface casing is shut in; the valve on the 4.5 inch is open. The line then goes to a drip and then there is a 2 inch plastic line leading underground. There is no other equipment tied to the well. No one affiliated with Tuco is claiming knowledge or ownership of the well. A Chiefs Order was drafted and sent to the Divisions legal staff. Derby Operating owns gathering lines in the area. Gary Miller of Derby said this well was originally tied in to their lines but never made gas and he doesnt believe it is connected anymore. He stated the landowner took over the well, however no documentation of that has been found. The property is vacant land and a Minneapolis based trust is the owner of record. It is unknown where the 2 inch line goes. The house closest to the well is not connected to it.
