0016776134 2018-08-20 Plug / Plug Back No Violations The operator requested inspection to witness plugging operations at the well. Cement was tagged in the tubing at 200 feet down. Cement was tagged outside the tubing at 120 feet. The annulus between the 1.5 inch tubing and 4.5 inch casing was filled solid with cement. The tubing was filled up to the top several times with cement by pumping down 1 inch plastic line installed in the tubing. Cement continued to fall down hole after pumping. The operator shut down for lunch and returned and attempted top up of the tubing again. Cement dropped following pumping. The operator is going to let cement set up overnight and attempt top up again in the morning.
0078862351 2018-08-22 Plug / Plug Back No Violations The operator is cutting off casings at 3 foot depth. No leakage observed from the well. Cement is at surface in the 1.5 inch tubing and 4.5 inch casing. The pit has been pumped down. The operator is going to remove the liner and restore the location. 38 bags of type 1 cement were pumped in the well in addition to 1.5 to 2 yards of 9 sack grout placed in the well by Moritz Materials.

0930274967 2015-10-30 Production Wells No Violations I spoke with Deep Resources personnel regarding plugging or repairing the well to bring it into compliance. Tom Benko, ODNR was on site. The service rig is currently on an injection well and will be moving to another injection well following servicing the one they are currently on. Inspector Benko has issued compliance notices on several injection wells for leakage and the operator is addressing those at higher priority than production wells. They are planning to plug the well but they dont have a timetable for doing so at present. Randy Matheny of Deep Resources was on vacation. John and Ethan of Deep Resources were the points of contact.
0962313787 2015-04-10 Production Wells Violations Noted The well is shut in with a swage on the production casing. Gas was bubbling up through fluid in the surface production annulus. The well is idle and unidentified. Harold Moravy stated in a previous report that Len at Deep Resources was planning to plug the well. Len is no longer with the company. The well needs to be plugged and the location restored by 10/23/15.

It is idle and equipped with 7 and 3 casing with a swedge in 3 casing. No production equipment on well or tank battery. Production records indicate well produced 589 barrels of oil in 2009. This well has not produced for years. Advised Len Rex to plug or produce well. Cloudy, 35 degrees.

1534168042 2018-08-10 Plug / Plug Back No Violations The rig is unable to move the tubing today. The well would not circulate through the tubing either. The operator contacted wireline companies to schedule cutting the tubing off to finish plugging the well. Wireline service is scheduled for Tuesday 8/14.
1534275067 2018-08-14 Plug / Plug Back No Violations JET Wireline made attempts to cut tubing and the rig attempted to pull it from the well. Tubing was shot at 400 feet and was still in cement. Cement was tagged in the 4.5 inch-tubing annulus at 200 feet. Tubing did not pull free after cutting at 200 feet. The rig crew unscrewed tubing and 3 joints were recovered. The tubing was re-installed. The rig crew ran 500 feet of 1 inch plastic pipe inside the 1.5 inch tubing. The operator is going to fill tubing and the annulus with cement to finish plugging the well.
1534771961 2018-08-17 Plug / Plug Back No Violations The operator requested inspection to witness well plugging operations. 20 sacks of type 1 cement was pumped in the well through 1 inch plastic tubing inside the 1.5 inch tubing. Circulation returned up the 1.5 inch tubing.

1932613736 2018-08-09 Plug / Plug Back No Violations The operator requested inspection to witness plugging the well. Tubing was ran in to 795 feet. After starting grout down the well, the rig operator attempted to raise tubing and became hung up. About 1.5 - 2 yards of cement was placed in the well. Tubing moved and rotated but the rig was unable to free it from the well. The well was circulated with fresh water and then stopped circulating. The operator is going to make additional attempts to free the tubing tomorrow.
