0158123356 2017-08-02 Production Wells No Violations Palmer Enterprises arrived to remove their boring equipment. They mixed bentonite gel to seal the bore hole and pumped fluid as the drill string was pulled out. The well was monitored during the operation. No fluid circulated from the well. The stream was checked with Ralph Shorts. The water was clear of sediment. Oil sheen was observed above the boom. Additional diapers were placed to soak up pooled oil in the creek. The boom and diapers will be left until the oil sheen is absorbed. Straw bales will be removed when the landowner gives permission to Palmer to enter the property.
0314937017 2017-09-15 Production Wells No Violations Palmer Enterprises completed the underground utility bore on the south side of State Route 302 past the well location. No issues were observed with the well.

1501681319 2017-08-02 Production Wells No Violations A call was received from Ralph Shorts stating that Palmer Enterprises was doing horizontal boring along State Route 302 near this well for AEP. He stated that the boring operation communicated with the well and pushed oil and clay out of the drive pipe and into a creek near the well. On arrival, Palmer Enterprises had installed straw bales in the creek to contain spilled material. Gray fireclay was found sprayed on the pump jack, around the drive pipe and down over the creek bank into the creek. Cloudy water extended about 350 feet downstream. About 5 gallons of material was released including a small amount of oil. The boring job was shut down. Emergency Response and Ohio EPA were notified via the duty officer. Vikki Bunting of Emergency Response arrived on scene and placed booms and absorbent pads in the creek to clean up and contain oil. Straw bales were left to capture sediment. Oil contaminated straw bales were removed by Palmer and taken to a disposal facility. The horizontal bore hole is going to be plugged when equipment is removed. Ralph is going to re-clay the drive pipe-surface casing annulus. The bore was at 24 feet deep and the drive pipe is at 19 feet. The bore was past the well when the blowout occurred. Palmer stopped drilling when the release was noticed. AEP is trying to get approval to complete the boring on the south side of 302.
1501866012 2017-08-04 Production Wells No Violations No Palmer Excavating or AEP personnel were on site. Straw bales and oil absorbent pads are still in the creek. The bales and pads may be removed. The water is clear and no oil pools were observed. Dan Taylor of AEP was contacted. He stated they plan to complete cleanup on Monday. The landowner was contacted and advised that the contractor and AEP will complete cleanup on Monday. Ralph Shorts checked the well. Gas pressure was inside the surface casing. No gas on the surface-drive pipe annulus. The well was pumped for an hour. The well produced brine and oil. No fresh water or boring fluids were pumped from the well. The surface casing seal is intact. The operator is going to replace clay that was blown from the surface-drive pipe annulus by the boring operation on Monday as well.
1502223003 2017-08-07 Production Wells No Violations Met on site with Ralph Shorts. Palmer Enterprises removed absorbent pads, straw bales and the oil boom from the creek prior to arrival. They also picked up pads around the well but did not clean up spilled mud. Dan Taylor of AEP was contacted and dispatched a crew to finish cleanup around the well. The mud was removed by Palmer personnel. Ralph replaced clay in the surface drive pipe annulus from surface until the annulus was filled. The well will be monitored to ensure clay remains at surface. There was gas pressure inside the surface casing; no gas on the annulus. The well was pumped. No fresh water, drill mud or fluid was found in fluid pumped from the well. The surface casing remains intact.
