0141299475 | 2017-07-18 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with vegetation and erosion control established in the affected area of the well pad. Addressing the well status- |
0470290146 | 2016-11-16 | Drill / Deepen / Reopen | No Violations | During my inspection the 5.5 inch diameter production casing had just been cemented. CNX was rigging down and preparing to haul everything off of this location and onto the CNX Switz 28 pad. |
0629207692 | 2016-11-07 | Drill / Deepen / Reopen | No Violations | During my inspection the Patterson 803 rig was drilling the lateral at 14739 feet. |
0754662645 | 2017-04-21 | Completion Testing | No Violations | Status check inspection on the well completion process found the well stimulation process complete and Deep Well Service tubing snubbing unit rigged up on the hole preparing to begin running in the hole with a 2.875 diameter work over string to drill out the completion plugs and condition the well bore in preparation for production. The well head is equipped with a blow out prevent owned by Oil States Energy, the well control device consists of (20 blind rans and (2) 2.875 diameter pipe rams. Mid Atlantic Service fluid pump truck is on location providing fluid pumping downhole circulation services with Rock Water flow back services providing well pressure control with the discharge fluid flowing into (2) 500 barrel capacity open top gas buster steel construction fluid tanks. All completion support equipment is situated on polymer containment and the well pad perimeter earthen berm serves as the primary containment. Multiple well are present on the well pad. |
0875986683 | 2016-08-14 | Casing String Witnessed | No Violations | On site to witness conductor pipe run and cemented in place, Express Energy drilled a 36 diameter hole to 100 and ran 100 of 30 casing and they cemented it in place with 275 sacks of Class A cement and we had 15 bbl. Of good cement returns at surface and it stayed up good. |
1215031056 | 2016-08-14 | Casing String Witnessed | No Violations | On site inspection to witness the conductor casing cement job found Express Energys top hole rig spudded the said well on 08/13/2016 and drilled a 36 conductor borehole to 100 and ran 100 of 30 weld connection conductor pipe. Express Energy Cemented the well using 275 sacks of class A cement with circulation established prior to cementing and the cement displaced to 85 using 70 barrels of fresh water. The rig encountered a fresh water zone at 45. |
1293452111 | 2016-11-16 | Casing String Witnessed | No Violations | During my inspection Patterson 803 drilling rig was running the 5.5 inch diameter production casing into the 8.75 inch diameter borehole. Allied Oilfield Services was on location preparing to cement the 5.5 inch diameter production casing. |
1426803446 | 2018-08-06 | Production Wells | No Violations | Status check inspection found 4 wells producing on the pad. The pad surface is of stone construction with a earthen perimeter berm, the perimeter valves are closed. The well head is equipped with a lower A and upper B section along with a production tree. The well is currently in production with the production valves open. The well head has riser pipes and valves with gauges present to monitor annular pressure on the A & B sections with 238 pounds per square inch pressure (psi) on the A section which monitors the pressure on the 13.375 diameter casing string (between the 13.375 and 9.625). The pressure on the B section which monitors the pressure on the 9.625 casing string (between the 9.625 and 5.5 diameter casing strings) is 870 pounds pressure. The 5.5 diameter production casing internal working pressure is 1,802 psi. The production flowline pressure is 1,118 psi. The well head assembly is equipped with 2 manual and one emergency shut down valve on the vertical section of the production tree followed by a single manual valve on the horizontal outlet section of the production tree. Also on location is 4 gas processing units along with a production facility tank farm consisting of 4 steel construction 400 barrel capacity tanks situated inside a polymer lined steel containment dike area. Proper well identification is posted at the well heads along with hazard and identification on the tank battery. At all critical hazard and muster points of the pad are fire extinguishers and emergency shut down master well control devices. A EQT midstream point is on the right front corner of the well pad. |
1477221972 | 2016-10-22 | Casing String Witnessed | No Violations | On site to witness running and cementing of 18.675 casing Patterson rig 803 was drilling contractor and they ran 710 of casing equipped with float shoe, Blackhawk packer and float collar and two baskets and seven centralizers and Allied Cementing cemented it in place with 1023 sacks of Class A cement and they had 30 bbl. Of good returns at surface. |
1477662867 | 2016-10-28 | Drill / Deepen / Reopen | No Violations | During the time of my inspection the Patterson Rig 803 was drilling at 5145 feet in the 12.25 inch diameter hole. No pollution violations present on location. |
1514945587 | 2018-01-02 | Production Wells | No Violations | On site inspection as to a response to a report of a 25 gallon fluid release on the EQT midstream production pad that is situated on a site pad that adjoins the main well pad. The pad is situated on the South West side of the Switz 5 well pad and was deplicted on the construction submittal plans in 2015. No product was visible on the earths surface and there was a roll off container on site with material place in it that had been recovered. Addressing the well, the drilling and completion process is now complete and the well is in prodcution along with 3 additional wells on the pad. There is a total of (4) gas processing units on the well pad that the wells produce into. Alos present is a production fluid storage facility consisting of (4) steel concruction 400 barrel capacity production tanks situated inside a polymer lined steel containment dike atrea. Proper identification is posted at multiple locations on site. |
1719001509 | 2016-11-09 | Drill / Deepen / Reopen | No Violations | During my inspection the Patterson 803 rig was drilling at a depth of 16,735 feet in the 8.75 inch diameter borehole. |
