| I inspected this well location pursuant to a request registered by a prospective buyer (Complaint # 4390) of the property. At the time of this inspection, I observed the following: a 3 by 3 square, masonry vault located in the backyard of the house at the referenced address. Within the vault were apparent well elements including a casing riser with wellhead and domestic supply connections. The vault was partially filled with water obscuring any details of the well elements. I did not observe obvious leakage of fluids or gas. The vault structure was covered with a wooden lid. At the time of this inspection, the house was unoccupied.
