0836939985 2013-07-15 Production Wells No Violations I inspected this well location pursuant to a request registered by a prospective buyer (Complaint # 4390) of the property. At the time of this inspection, I observed the following: a 3 by 3 square, masonry vault located in the backyard of the house at the referenced address. Within the vault were apparent well elements including a casing riser with wellhead and domestic supply connections. The vault was partially filled with water obscuring any details of the well elements. I did not observe obvious leakage of fluids or gas. The vault structure was covered with a wooden lid. At the time of this inspection, the house was unoccupied.
1291077136 2015-12-03 Production Wells No Violations I inspected this well location as a follow-up to an earlier inspection which had been at the request of a potential buyer. At the time of this inspection, I observed the following: A square masonry well vault with wooden cover located in the back yard area. Within the vault is a wellhead with domestic valve connection. There is a sump pump type system in use to remove water from the vault interior. At the time of this inspection, the current residents are not home. As this well is not currently registered, I will have ODNR administrative support staff send a well registration package to the land owners.
1550157517 2019-02-14 UrbanProduction Wells No Violations I conducted an inspection of this well location. During my inspection, I observed the well to be located within a masonry vault with a wooden cover. The well was located behind the home. The well was setup with a domestic valve connection, and at the time of this inspection, completely submerged under water. There was a sump pump type system in use to remove water from the vault interior. The current residents were not home, at the time of this inspection. The well has not been registered with the state of Ohio and needs a well registration package sent to the land owners.
