0060519879 2016-08-07 UrbanProduction Wells No Violations I conducted an inspection on this well. The well was equipped with a plunger lift unit and the valves on the wellhead were open. The well was fenced and the man door was locked. The tank battery consisted of one, 100 BBL production tank, one, 50 BBL steel brine tank, a heater treater separator, and a mechanical separator. There was a 55 gallon charcoal filter plumbed into the 100 BBL production tank. There was lightning protection at the tank battery. The tank battery was diked and fenced and the man door on the fence was locked. The well was identified with a clear, legibly marked well identification sign located on the tank battery fence. The access was stable. No violations.
0248102775 2011-04-13 Production Wells No Violations Received call from Geauga EMA, oil spill at well. Well has been spraying oil into the air possibly all night. Meet John Warelin Chester TWP. Fire Chief on location. Rabbit catcher broke off well at approximately 7:00am, fire department was on scene at 7:45am, well was shut off. Demco valve was in place, FDP was able to use valve to shut well down. Inspector on site at 8:50am. Fire Department was putting in place booms and diapers. Rabbit catcher broke off of the lubricator nipple. Rabbit is threaded onto the 1.5 inch nipple, rabbit broke off at the threads. (Pictures taken) The well sprayed approximately 2Bbls of oil and brine. The area affected by the spray was at the furthest extent 300 feet east and west in a cone shape from the wellhead and 600 feet south of the wellhead. Grossly impacted soil area extent was 75 feet south spreading out in a cone shape 85 feet east and west. Oil was released into a tributary of the Chagrin river. Approximately 10 gallons of oil was on a pond that discharges into the tributary. Operator started the following actions; Stop additional oil from entering waterway. Remove free product from water area. Remove grossly impacted soil and vegetation. Mix peat sorb into lesser impacted soil. By 3:30pm operator had completed above tasks. Operator dammed off a portion of the pond and installed a siphon dam and booms to absorb product residue. Operator had used a vac truck to remove product from pond. Operator had scrapped up grossly contaminated soil and removed the majority of it. Operator began topping driveway with new limestone. Broke equipment was removed.

The tank battery and well are in good repair and surrounded by a chain link fence. The tank battery consists of a 100 BBL production tank, a 50 BBL brine tank, and a mechanical separator. The vent stack on the production tank is plumbed into a 55 gallon charcoal filter. The vent stack is then plumbed up and topped with a spark arrestor. The proper identification is located on the tank battery. The lease road is in stable condition.

Operator failed to contact inspector 24 hours before pit construction. East pits had been constructed that day, they were not lined. Water was entering pit from the north. Operator packed the sides of the pit with clay, and sprayed the sides with a bentonite slurry.
