| I conducted a final restoration inspection.  The well was plugged on 12/05/2016 and a plugging report is on file.  The tank battery was shared with permit numbers (20364, 20365)  which are still in production so the tank battery was not removed.  The access road  remains and is shared with the Downes #2 well (#20365).  All production equipment and debris has been removed at the well head. The area has been graded, but vegetation has not yet been established.  Final restoration can be passed once vegetation has been established.

| I conducted a compliance notice follow--impacted soil.  The tank battery now consists of one-210 bbl production tank which is equipped with proper plugs on the valves.  The leaking heater treater and brine tank have been removed, along with the two unused 100-bbl production tanks.  All oil and brine impacted soil has been removed.  The dike surrounding the tank battery has been repaired and re-built, and the tank battery is now separated from the Petrox tank battery. Proper identification has been posted at the tank battery.  The remaining issues that need addressed in order to close the compliance notice are to post proper identification at the well, remove and properly dispose of all contaminated soil around the wellhead, and to determine the source of the leak so that it can be repaired of immediately.

| I conducted a compliance follow-up inspection of the well and tank battery.  The belt guard had been installed on the pump jack.  The 8 and 5/8 casing head was exposed so that the source of the leak could be determined, but the well appears to still be leaking as there was active bubbling coming up through the standing water at the well head.  The oil-stained soil and previous fresh oil had been removed, however, there was new, fresh oil accumulating in the standing water around the well head.  Inspection of the tank battery showed that the unused production tank located north east of the dike, the horizontal separator,  and leaking heater treater inside of the tank battery had been removed.  The tank battery now consist of one, 210 barrel tank. The brine and oil impacted soil had been excavated out and removed.  Construction of a new dike was started, but appears to be unfinished as there are breached and incomplete areas of the dike.  No identification was posted at the tank battery.

| On site to conduct a follow up inspection on  a compliance notice issued by Inspector Ashley Raff for the well leaking gas and oil. Lakeside dug around the well head to determine the source of the leak. A triangle of cement, that was evident around the well prior to excavation, has been totally exposed. The only casing that is visible under the exposed triangle of cement is conductor casing.

| I was on location to observe the pumping of the above well. The well was pumped for 2 hours on by-pass and the 4.5 casing values were shut there was 72 psi. on the casing. There was no oil or gas seen coming from the 8.625. It appears the oil and gas is released as it pressures up inside the annuals. This may not be related to a 4.5 problem. Dave  field man for G&O will talk to his supervisor to see what they are gone to do to repair this problem.

| Moore Well Service started pulling the 2-3/8 tubing. The first 1500 tubing was dry then it change to a wet string. We shut down until we got a pit dug and lined it with a 40 mil liner. After a few more joint of 2-3/8 tubing it turned to a paraffin base oil. Tubing was removed well packed in over night. Sutton Pump and Supply will be on location to steam tubing and remove the oil and paraffin from location.

| I meet with ODNR Inspector Bob Roush and ODNR Inspector Ashley Raff , Dave Wittick from G&O, Ron Hammond from Lakeside Exv. To discuss the remove of a concrete pad around the wellhead. The well is leaking some oil and gas it looks to be coming from between the 8.625 casing hanger and the 4.5casing. They pumped the well and it showed some oil coming from the annuls. The 4.5 casing pressured up to 80 psi. The pump was shut off. The pressure did not drop to zero but stayed at 70 psi. 4.5 casing integrity looks good at this time. The decision was made to leave the concrete around the well until upper management returns from vacation.

| I conducted a status check of this well with DOGRM inspector Heidi Scott.  Our inspection found this well to be equipped with an electrically-driven pumpjack.  The polish rod was not lubricated.  Fresh oil measuring 88 inches by 77 inches surrounded the wellhead, and there was active bubbling through the oil at time of our inspection.  The tubing valve coming out of the wellhead was open.  We were unable to determine if the production casing valves were open at the time of our inspection because they were coated with fresh oil.  Inspection of the tank battery found that it consisted of one-210 bbl production tank, one-210 bbl tank labeled as a brine tank, horizontal separator, a meter run, and a heater treater separator.  There was also an unused 100 bbl tank with no valves, and an unused 100 bbl tank that was located outside of the dike.  This well shares a tank battery with G &O wells PN # (364, 365, and 383). This pad is shared with Petrox tank battery PN # (1102 and 1103). There was an oil stain around the separator measured at eleven inches by ten inches.  The brine line at the back of the 210 bbl tank was leaking brine, and had formed a puddle measured at 32 inches by 17 inches. At the time of inspection, the brine was dripping at a rate of 2 drips per second, and there were salt crystals present under the brine line. There was four inches of standing water adjacent to the heater treater inside of the dike with two inches of oil on top of the water. The dike was heavily eroded and there was evidence that it had been previously breached.  Weathered oil was present outside of the dike, and oil stained soil was present on the haul road. The remaining issues that need addressed in order to close the compliance notice are to post proper identification at the well, remove and properly dispose of all contaminated soil around the wellhead, and to determine the source of the leak so that it can be repaired of immediately.

1337194263 2012-05-16 Production Wells Violations Noted My inspection found pollution at the well head and natural gas bubbling below the 4.5 inch casing. I called the number listed for G & O Resources Ltd (330-761-3508) and it went to a fax line. I then called the emergency number listed on the well identification and spoke with Dave Widuck (330-323-6732) and he advised me he would have it cleaned up with in the week. The record shows the well has reported production for 2010 and no other violations are noted at this time.

1429727313 2015-03-31 Production Wells Violations Noted I conducted a status check of this well with DOGRM inspector Heidi Scott. Our inspection found this well to be equipped with an electrically-driven pumpjack. The polish rod was not lubricated. Fresh oil measuring 88 inches by 77 inches surrounded the wellhead, and there was active bubbling through the oil at time of our inspection. The tubing valve coming out of the wellhead was open. We were unable to determine if the production casing valves were open at the time of our inspection because they were coated with fresh oil. Inspection of the tank battery found that it consisted of one-210 bbl production tank, one-
1435082140 2015-06-23 Production Wells No Violations I conducted a compliance follow-up inspection of the well and tank battery. The belt guard had been installed on the pump jack. The 8 and 5/8 casing head was exposed so that the source of the leak could be determined, but the well appears to still be leaking as there was active bubbling coming up through the standing water at the well head. The oil-stained soil and previous fresh oil had been removed, however, there was new, fresh oil accumulating in the standing water around the well head. Inspection of the tank battery showed that the unused production tank located north east of the dike, the horizontal separator, and leaking heater treater inside of the tank battery had been removed. The tank battery now consist of one, 210 barrel tank. The brine and oil impacted soil had been excavated out and removed. Construction of a new dike was started, but appears to be unfinished as there are breached and incomplete areas of the dike. No identification was posted at the tank battery.
1440181421 2015-08-21 Production Wells No Violations On site to conduct a follow up inspection on a compliance notice issued by Inspector Ashley Raff for the well leaking gas and oil. Lakeside dug around the well head to determine the source of the leak. A triangle of cement, that was evident around the well prior to excavation, has been totally exposed. The only casing that is visible under the exposed triangle of cement is conductor casing.

Lakeside had removed the contaminated dirt around the well head and the pit around the well is fairly clean of oil. The contaminated dirt is in a stockpile next to the well and Lakeside would like to mix it with manure and spread it near the well. -? leak is still not evident due to the cement triangle being intact, except for the hole at the bottom of the triangle right above the conductor casing. This hole is on the left side of the triangle (s.) if you are facing the pump jack. This is also the side where the gas was bubbling prior to excavation.

Further removal of the cement triangle to determine the source of the oil/gas leak will take place next week.

1441029516 2015-08-27 Production Wells No Violations I was on location to observe the pumping of the above well. The well was pumped for 2 hours on by-pass and the 4.5 casing values were shut there was 72 psi. on the casing. There was no oil or gas seen coming from the 8.625. It appears the oil and gas is released as it pressures up inside the annuals. This may not be related to a 4.5 problem. Dave field man for G&O will talk to his supervisor to see what they are gone to do to repair this problem.

1482932636 2016-11-29 Plug / Plug Back No Violations Moore Well Service ran 5260 of 2 tubing in the well to spot the bottom plug. Rob Lowe from our survey group was on location to get the well head location. Tubing was landed and the well packed off and shut in over night.

1569165864 2016-11-22 Production Wells No Violations Moore Well Service started pulling the 2-3/8 tubing. The first 1500 tubing was dry then it change to a wet string. We shut down until we got a pit dug and lined it with a 40 mil liner. After a few more joint of 2-3/8 tubing it turned to a paraffin base oil. Tubing was removed well packed in over night. Sutton Pump and Supply will be on location to steam tubing and remove the oil and paraffin from location.
2040725744 2015-08-24 Production Wells No Violations I meet with ODNR Inspector Bob Roush and ODNR Inspector Ashley Raff , Dave Wittick from G&O, Ron Hammond from Lakeside Exv. To discuss the remove of a concrete pad around the wellhead. The well is leaking some oil and gas it looks to be coming from between the 8.625 casing hanger and the 4.5casing. They pumped the well and it showed some oil coming from the annuls. The 4.5 casing pressured up to 80 psi. The pump was shut off. The pressure did not drop to zero but stayed at 70 psi. 4.5 casing integrity looks good at this time. The decision was made to leave the concrete around the well until upper management returns from vacation.
