0105465262 2018-07-24 Completion Testing No Violations Status check inspection accompanied by ODNR Inspector Ryan Armstrong found the well drilling and stimulation process complete. The well head master valves used during the completion process are still in place with a manual and hydraulic master valve present. A coil tubing unit owned by Cudd Services is scheduled to move onto the well site and rig up on the hole in the next 24 hours. There is a total of 43 completion plugs to drill out. Annular pressure monitoring is in place with zero pressure on the 13.375 well head section (between the 9.625 and 13.375 and zero pressure on the 9.625 well head section between the 5.5 and 9.625 casing strings. The well pad is equipped with polymer containment on the surface.
0480472094 2018-01-18 Drill / Deepen / Reopen No Violations On the day of my inspection I found Patterson 296 had just skidded this morning. The 9.625 inch diameter casing was previously set at 8313 feet. They will be ging back in the hole with 8.75 inch diameter bit tonight. Containment is functioning, one section of berm approxzimately 10 feet long was repaired while I was onsite. This is the containment below the rig and mats not a pad berm.

There is 3 wells on the pad that were recently drilled with 2 stimulated and the 3rd and final well now being stimulated. The company is monitoring all annular pressures.
0827209561 2018-08-08 Completion Testing No Violations On the day of my inspection I witnessed that the well was setting static with annular pressure monitoring in place. After speaking with company man Matt Bradford, he informed me that the all 43 isolation plugs on the well had been drilled out and the well was waiting to commence flowback operations. Mountain State Pressure Systems was currently on location drilling out isolation plugs on 34013211820000. There were 4 producing wells on location that were covered with steel cages.

1508869525 2017-10-24 Casing String Witnessed No Violations On site inspection to witness the conductor casing cement job accompanied by ODNR Inspector Andrew Corder. On this day it was observed that Express Energys top hole rig has drilled a 36 diameter conductor borehole to a measured depth of 100 and inserted 100 of 30 diameter .375 wall weld connection casing. Express Energys Cement Service cemented the casing using a 57 barrel absolute volume cement mix consisting of 275 sacks of class A cement with the cement weight verified at 15.6 pound per gallon by weight of scale and the cement displaced with 75 barrels of fresh water. Cement returns were witnessed to surface inside the cellar.

1516720273 2018-01-22 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 296 driling an 8.5 inch diameter lateral hole section at a depth of 13610 feet. Containment is functioning and repaired berm viewed in last inspection is still functioning. Small flare burning occasionally on location.
1527171247 2018-05-24 Completion Testing No Violations Status check follow up inspection to an unreported incident that occurred on 05/11/2018 at 18:01 pm involving the said well. My investigation found that on 5/11/18 the company was stimulating stage 9 out of a proposed 44 stages to stimulate and was 60% complete with the stage and pumping at a rate of 100.5 barrels per minute with a treating pressure of 10,300 pounds per square inch when a spacer spool in the 7.125 completion well head assembly failed. The Silver Creek companys completion stack was situated on top of the B section of the GE (General Electric) well head assembly and the spool was situated between the lower manually operated master valve and the hydraulic master valve. The weld on the spool failed allowing the completion assembly to separate and fall off the well head. A worker was able to access the manually controlled lower master valve and control the well flow. The company man on site reported a undetermined amount of well bore related product was dispersed onto the polymer containment and a total estimated 2 barrels of fluid lost off the polymer containment and onto the well pad. The company man on site identified as Walt Arnold indicated the product on containment was recovered and the liner removed and replaced and the area off containment but on the well pad was remediated by removing the affected material and recovering any fluid with a vacuum truck. There was a small amount of standing fluid in a depression on the well pad that was titrate tested for chlorides with a low level Quantab strip test and found to exceed the low level strip test reading of 637 parts per million, I also titrate tested the same area with a high level Quantab strip test and found 751 parts per million chlorides. Directions were given to the company man on site Walt Arnold to continue to recover any surface water on the pad from this area. All perimeter drain valves were reported as closed. 1541659210 2018-07-24 Completion Testing No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that there was a completion stack on the well head with a manual master valve and hydraulic master valve present. Annular pressure monitoring was in place and the 13.375-inch diameter casing had 0 pounds per square inch and the 9.625-inch diameter casing had 0 pounds per square inch, the 18.625-inch diameter casing was also being monitored and was at 0 pounds per square inch. After speaking with EQT company man he informed me that they would be moving a CUDD coil tubing unit on the well on 7/25/2018 and would proceed in drilling the isolation plugs.
1922062354 2018-08-08 Completion Testing No Violations On site inspection accompanied by ODNR Inspector Ryan Armstrong. Currently there are 4 producing wells on the pad and 3 new wells that the company has service crews on location and completing. The said well is equipped with a lower A section and upper B section with a completion stack present owned by Cameron well company, the well will initially produce through this stack. The production equipment includes 4 gas processing units and 2 glycol heaters. Upon entering the location on the left side of the well pad is the production fluid facility consisting of 6 steel construction 400 barrel fluid capacity production tanks situated inside a polymer lined steel containment dike area. The well has had the completion plugs drilled out with a total of 46 plugs set during the completion process. No tubing will be set in the well only a tubing hanger. The company is monitoring annular pressure.
2042089368 2019-01-24 Production Wells No Violations On site inspection found the said well now in production and selling off the 5.5 diameter production casing through (2) primary master valves. The casing pressure is being monitored off a electronic transducer. No annular pressure monitoring is in place. 2 Crew Men from EQT are on site replacing the emergency shut down master valve hydraulic fluid. 3 wells were recently drilled and completed and 4 pre existing wells. Production equipment consists of 5 gas processing units along with a (6) steel 400 barrel capacity steel tanks situated inside a polymer lined steel containment dike area. The well pad perimeter is protected by an earthen berm however one section was removed near the mid stream equipment and not put back. At the rear of the pad is the mid stream operation operated by EQT. I contacted ODNR Engineer Technician Anthony Carson and he will address the issue with the perimeter berm. We have addressed this issue one before and it was addressed. The Preliminary Restoration is due however recent disturbance around the well pad caused by mid stream installation shall delay the restoration until proper vegetation is established. The well pad and lease road surface is stone and in good repair.
