Annular pressure monitoring is in place with zero pressure on the 13.375 well head section (between the 9.625 and 13.375 and zero pressure on the 9.625 well head section between the 5.5 and 9.625 casing strings. The well pad is equipped with polymer containment on the surface. |

Express Energys Cement Service cemented the casing using a 57 barrel absolute volume cement mix consisting of 275 sacks of class A cement with the cement weight verified at 15.6 pound per gallon by weight of scale and the cement displaced with 75 barrels of fresh water. My investigation found that on 5/11/18 the company was stimulating stage 9 out of a proposed 44 stages to stimulate and was 60% complete with the stage and pumping at a rate of 100.5 barrels per minute with a treating pressure of 10,300 pounds per square inch when a spacer spool in the 7.125 completion well head assembly failed. The Silver Creek companys completion stack was situated on top of the B section of the GE (General Electric) well head assembly and the spool was situated between the lower manually operated master valve and the hydraulic master valve. The weld on the spool failed allowing the completion assembly to separate and fall off the well head. A worker was able to access the manually controlled lower master valve and control the well flow. The company man on site reported a undetermined amount of well bore related product was dispersed onto the polymer containment and a total estimated 2 barrels of fluid lost off the polymer containment and onto the well pad. The company man on site identified as Walt Arnold indicated the product on containment was recovered and the liner removed and replaced and the area off containment but on the well pad was remediated by removing the affected material and recovering any fluid with a vacuum truck. There was a small amount of standing fluid in a depression on the well pad that was titrate tested for chlorides with a low level Quantab strip test and found to exceed the low level strip test reading of 637 parts per million, I also titrate tested the same area with a high level Quantab strip test and found 751 parts per million chlorides. Directions were given to the company man on site Walt Arnold to continue to recover any surface water on the pad from this area. All perimeter drain valves were reported as closed. The casing pressure is being monitored off a electronic transducer. No annular pressure monitoring is in place. 2 Crew Men from EQT are on site replacing the emergency shut down master valve hydraulic fluid. 3 wells were recently drilled and completed and 4 pre existing wells. Production equipment consists of 5 gas processing units along with a (6) steel 400 barrel capacity steel tanks situated inside a polymer lined steel containment dike area. The well pad perimeter is protected by an earthen berm however one section was removed near the mid stream equipment and not put back. At the rear of the pad is the mid stream operation operated by EQT. I contacted ODNR Engineer Technician Anthony Carson and he will address the issue with the perimeter berm. We have addressed this issue one before and it was addressed. The Preliminary Restoration is due however recent disturbance around the well pad caused by mid stream installation shall delay the restoration until proper vegetation is established. The well pad and lease road surface is stone and in good repair. |
